The Immunotherapy Toxicity Management Program (iTox) Consult Physician Assistant (“iTox Consult PA”) will be part of the solid tumor consult team at Dana-Farber and will work closely with ambulatory and inpatient iTox teams. The primary responsibility of this role will be working with the iTox consult attending physicians to staff and manage inpatient iTox consults. The iTox Consult PA will serve as the key contact for iTox consult requests and will partner closely with the iTox attending and the requesting team to communicate recommendations and support care coordination. They will continue to follow consult patients through their admission until discharge, including follow-up on clinical data, coordination of care, and communication with patients and families to support continuity of care. This role requires practice across two locations, with travel between sites expected during work hours.
The iTox consult PA will be cross-trained to cover the solid tumor consult service and will work closely with other solid tumor consult PAs to support cross-coverage and shared learning. In addition to core iTox consult responsibilities, the iTox Consult PA may also participate in other oncology consult services, ambulatory management of patients followed by the iTox program, and/or inpatient management of patients with immunotherapy-related toxicities. The scope of this role is expected to continue to evolve in alignment with Dana-Farber’s transition to the new Lavine Bekenstein Cancer Hospital at Dana-Farber.
Professional collaboration between inpatient and the ambulatory setting is expected by all PAs.

- Manages all aspects of patient care in collaboration with his/her supervising MD
- Examines patients to obtain information about their physical condition
- Orders and interprets diagnostic test results to obtain pertinent medical information
- Obtains, compiles, and records patient medical data which may include previous notes, consultations, treatment flow sheets, and laboratory, radiology and pathology data
- Prescribes therapy or medications and performs procedures according to DFCI privileges, this may include chemotherapy ordering in a medical oncology practice
- Demonstrates ability to work collaboratively with all members of the care team and all DF/BW staff to promote excellence in patient care
- Responds appropriately to difficult situations, personalities, and is able to maintain composure and work with all members of the healthcare team
- Attends disease center staff meetings, tumor boards, and conferences. Completes required training/PA program activities
- Practices in an efficient manner without compromising high level of patient care
- Completes documentation of notes based on standard practice of institute
- PA will always ensure coverage for themselves when they are out of the hospital
- The PA will work collaboratively with MD on new patient visits to establish MD/PA team concept and expectations with patient early in care
- At 3 months (earlier if applicable), the PA will begin to see patients independently and will develop and maintain an independent schedule. 30 – 32 hours per week of scheduled clinical time is expected with remainder of the time for administrative tasks/documentation/patient follow-up and coordination of care
- At 6 months (earlier if applicable), the PA will provide coverage for MD when he/she is out of the hospital. They will see acute/walk-ins independently, cross cover for NP/PA disease center team
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Expert knowledge of general medicine and hematology/oncology. Ability to incorporate knowledge of general internal medicine and medical complications unique to oncology patients in day-to-day management of patients
- Must be professional, quality driven, and have the ability to work well within a cohesive group of peers. Exercises highest level of professional judgment and will apply independent originality in solving programs
- Strong communication skills
- Possesses maturity and confidence to work closely with clinicians, therapists, and team members across the organization. Demonstrates ability to work collaboratively with all members of the care team and DF/BWH staff
- Demonstrates a commitment to ongoing clinical education and training
- Able to accept instruction and guidance and assume graded responsibility in complex teamwork, while demonstrating an understanding of procedures, policies, and documentation required to facilitate PA compliance with standards of care
MINIMUM JOB QUALIFICATIONS:
The position requires a master’s degree and completion of an AAPA/NCCPA-accredited Physician Assistant Studies program. Candidates must be NCCPA certified, licensed to practice as a Physician Assistant in Massachusetts, and maintain current DEA and Massachusetts Controlled Substance Licenses. Additionally, 100 hours of CME must be completed every 2 years in accordance with national and state regulations.

Job Summary:

Position Purpose
As a valued member of Trinity Health At Home, the Homecare Certified Nursing Assistant (CNA) provides compassionate, paraprofessional care to patients in their homes under the direction of a Registered Nurse and in accordance with physician orders. Guided by the state Nurse Practice Act and agency standards, you will support clients with essential daily care, promote safety and comfort, and help them maintain dignity and independence in their home environment.
What You Will Do
Deliver personal care services such as bathing, grooming, skin care, dressing, toileting, transferring, and ambulation following the RN‑developed care plan.
Assist with household tasks including meal planning and preparation, feeding, light housekeeping, laundry, and bed‑making.
Measure and record vital signs including temperature, pulse, respiration, and blood pressure as ordered.
Support therapy services by performing assigned range‑of‑motion exercises and other simple procedures.
Assist clients in self‑administration of medications as assigned and documented by the RN (CNA does not administer medications).
Provide support with medical equipment such as wheelchairs, side rails, and footstools to ensure safety.
Perform personal care services such as changing ostomy appliances or external catheter care per the care plan.
Report changes in client condition or incidents immediately to the supervising RN.
Maintain accurate, timely documentation and collaborate with schedulers to ensure an up‑to‑date care schedule.

Minimum Qualifications
High School diploma or GED.
Completed a federally compliant CNA or aide training program and passed a competency evaluation.

Ability to work independently while maintaining professional relationships with clients and families.

·Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.

Demonstrate knowledge and ability to facilitate living donation/transplantation and will utilize that knowledge to coordinate the care of the living donor for evaluation and live donation. Identifies appropriate live donor candidate upon referral and interacts with appropriate staff.
Demonstrates knowledge and ability to coordinate the care of the transplant recipient in the outpatient setting to achieve optimal physical, social and emotional rehabilitation. Acts as a liaison between the donor and recipient transplant teams. Exhibits ability to remove transplant recipient from list per regulations. Provides outcome specifics to regulatory bodies for donor family follow-up. Maintains communication with referring physician, health care providers and payers.
Identifies and responds to the psychosocial and economic needs of the transplant candidate/family/legal guardian during evaluation. Demonstrates knowledge of, and compliance with, policies and listing requirements. Informs and educates patient/family/legal guardian at time of listing. Ensures patient and referring physician are informed in writing of transplant listing, denial or postponement. Provides and maintains comprehensive documentation of evaluation process and makes appropriate referrals to specialists.
Interprets serological results of the potential transplant candidate and their implications for transplant. Obtains recipient blood type, tissue typing and cytotoxic antibodies according to transplant center protocol. Identifies possible co-morbidities in the transplant candidate and evaluates as indicated. Identifies absolute and relative contraindications to transplantation. Ensures patient understands risk and benefit of immunizations required prior to transplantation.
Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

$41.10 - $61.65Major Responsibilities:
·Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
·Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
·Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
·Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
·Completes all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits.
·Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
·Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise.
·Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
·Pursues professional growth through participation in continuing education courses, department inservices, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
·Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. POSITION SUMMARY
This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS
1. Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflects the key limiting factors. Collaborates with patient and family/caregiver when setting goals. Initiates discharge planning. Recommends additions to or modifications of referring orders.
2. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
3. Documents according to professional practice guidelines set by regulatory agencies. Documents legibly. Completes documentation within the established time frames of the department. Documents evidence based and individualized treatment techniques/plan. Documentation reflects skilled intervention and comprehensive assessment of the patient's progress or lack of progress. Treatment goals are reassessed according to department policies.
4. Communicates pertinent patient information to ensure the best possible follow through of care. Communicates evaluation/treatment results and patient progress to physicians, nurses, patient, family/caregiver, and interdisciplinary team. Communicates in a clear and timely manner. Accepts responsibility to contact physicians for routine or difficult problem solving to ensure optimal patient outcome. Defends treatment rationale with standard practices and/or current literature.
5. Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing shifts. Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs.
6. Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality cost effective patient care. Supervision and delegation of the PTA, ATC, Therapy Technician and student complies with department policies and regulatory agencies. Participates in the training and orientation of new personnel. Assures competency of personnel, prior to delegation of task.
7. Department operational activities. Attends department required meetings and/or follows up to obtain pertinent information. Participates in departmental/facility process improvement. Meets productivity standards set by the department.
8. This position has responsibility for interacting with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community provides and agencies concerning the health care of the patient. Also interacts with the physician offices in order to report, request or clarify information.
MINIMUM QUALIFICATIONS
Must have graduated from an APTA accredited school of Physical Therapy, or equivalent.
Must possess a current Physical Therapy license within the State of practice. Per California regulations, internationally trained candidates have six-months from the time of hire to obtain a current license.BLS certification required.
Relates throughout the interview process their clinical competency, experience, training and education they have received that is required to perform the job. Demonstrates through behaviors and answers to job related experiential questions. Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment. Patient care equipment, evaluation tools, and therapeutic modalities. Copier, multi-line feature telephone, fax machine, computer, pager, and miscellaneous office equipment.
