FLOAT POSITION INCLUDES A $5.00 PER HOUR DIFFERENTIAL AND $3.00 WEEKEND DIFFERENTIAL.
OFFERING A $5000 SIGN-ON BONUS PAYABLE IN TWO INSTALLMENTS - 50% AFTER 90 DAYS - 50% AFTER ONE YEAR
Teammate will FLOAT TO OUTPATIENT Advocate Health areas within 40 minutes of Advocate Sherman.
Pay Range:
$41.10 - $61.65Major Responsibilities:
·Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
May supervise physical therapy assistants following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. May supervise physical therapy extenders (rehab aides/techs) in administering treatments to patients and will teach them appropriate techniques to perform with specific patients. May supervise physical therapy students after completing one year of clinical experience.
Minimum Job Requirements
Education (minimum education level and field of study)
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License (any certification, registration, license required to perform the job followed by the governing body)
Physical Therapy License in the State of Illinois or State of Wisconsin, must be licensed in the State upon which you work. Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)

Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.

Requirements

- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred

Minimum Job Requirements
Education
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.

- Daily, provides ongoing Medical Nutrition Therapy (MNT) care for entire hospital inpatient census, through screening, prioritizing, assessing, and recommending interventions. Adapts daily workflow to accommodate fluctuations in patient load due to daily census variations, consults, acuity, staffing challenges. Develops and implements a plan of care for patients based on an expert assessment of life cycle- and diagnosis-specific nutritional needs. Assessment includes reviews of medical history and laboratory data, patient interviews, and physical examinations to determine the current nutritional status of inpatients and/or outpatients. Based on the assessment, the Clinical Dietitian develops and implements individualized nutrition plan of care interventions. Documents medical nutrition therapy plan of care in the electronic medical record supporting system clinical documentation and coding standards to maximize reimbursement rates.
- Prescribes enteral and parenteral nutrition support orders by assessing the patient and calculating macronutrients and/or micronutrients, fluid, and electrolytes. Evaluates patient tolerance and effectiveness of formulas and manages initiation or modifications to enteral/parenteral regimen orders. The Clinical Dietitian demonstrates extensive knowledge of specialized nutrition support (parenteral and enteral) and triages issues and concerns through collaboration with physicians and pharmacy. Constant, secure communication with system level Pharmacy Integrated Clinical Services (PICS) team to manage and order Total Parental Nutrition (TPN) infusions for critically ill patients in the hospital.
- The Clinical Dietitian prescribes orders for therapeutic diets, oral nutrition supplements, and nutrition support regimens, including vitamins/minerals, fluid management, and lab work as indicated within the scope of the Medical Nutrition Therapy (MNT) protocol. Utilizes order-writing privileges to implement nutrition related orders. Collaborates with Food and Nutrition patient services team to ensure patient intake aligns with nutrition prescription and interdisciplinary plan of care. Partners with site patient services team to identify and recommend appropriate clinical nutrition formula and supplement substitutions during supply chain disruptions to reduce and eliminate significant disruptions to patient care. Partners with physicians, nursing, and nutrition patient services to ensure diet restrictions, allergy restrictions, supplement preferences and tube feedings are consistent with patient needs and personal, religious, and cultural preferences. Develops patient-specific menus for patients admitted with complex dietary needs (e.g., multiple food allergies, numerous diet restrictions, and/or low-use, high risk diet orders).
- Leads efforts to identify, treat and prevent malnutrition. Performs Nutrition Focused Physical Examinations of high-risk patients to evaluate the loss of specific muscles and subcutaneous body fat to determine the presence of and degree of malnutrition. Communicates findings to the medical team through medical record documentation to ensure maximized reimbursement. Provides medical nutrition therapy education to patients identified with moderate or severe malnutrition to improve patient outcomes and to positively impact inpatient readmission rates.
- Routinely documents elements of the Global Malnutrition Composite Score (GMCS) for Medicare and Medicaid Services’ (CMS) Nutrition Electronic Quality Indicators. Participates in Quality Assessment Performance Improvement (QAPI) efforts to ensure nutrition care is safe and effective for patients. Maintains regulatory readiness. Supports department clinical initiatives to ensure compliance with safety initiatives, policies, and procedures, DNV regulations, and department standards.
- Identifies and documents billable nutrition diagnosis with supporting documentation. May provide clinical support to various hospital-based outpatient medical destination programs (e.g., cancer center, transplant clinic, digestive health programs, bariatric, cardiac rehab, heart failure, etc.) Enters charges for outpatient nutrition charge capture in electronic medical record.
- Supports partnerships with local didactic programs by precepting dietetic graduate students in clinical nutrition and/or foodservice management. Directs and monitors interns’ progress both daily and at designated touchpoints and provides feedback for growth and development. Communicates regularly with didactic program coordinator. Also, precepts medical residents and provides nutrition orientation as applicable. Provides nutrition lectures to medical residents for internal medicine, trauma, NICU/Peds, and family practice, as applicable.
- Collaborates routinely with discharge planners, home health, physicians, pharmacy, and other interdisciplinary teams to address the full scope of the system-established priority points protocol for inpatient care. Participates in interdisciplinary team rounds, meetings, and care conferences to coordinate and collaborate on nutrition goals. Assists with transition to home or outside facilities by partnering with Care Management and/or Home Health to coordinate home enteral and/or parenteral nutrition needs.
- Provides medical nutrition therapy education to patients on prescribed diet, per established priority system, to improve patient knowledge/understanding of nutrition, diet therapy and compliance on prescribed diet. Education is provided to inpatients, outpatients and family members through individual counseling and classes. May provide nutrition in-services to teammates, as applicable.
- Provides clinical input into system clinical nutrition initiatives and practice changes. Participates in system subgroups/projects, as assigned, to develop and implement evidence-based nutrition practices, clinical initiatives and/or practice changes. Partners with system patient services and clinical teams to deploy system changes and updates regarding CBORD, EPIC, menus, emergency preparedness, downtime processes, formulary, clinical procedures, and policies. Liaisons between clinical nutrition and inpatient nursing and physicians to champion system clinical nutrition initiatives (e.g., new dysphagia standards, malnutrition coding practice changes, enteral nutrition equipment changes, malnutrition nurse screening changes, safety protocol changes, etc.) Assists (via in-person or remote coverage) other sites throughout the State, as needed, to provide clinical nutrition coverage to assure equitable patient care.

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

The Speech-Language Pathologist collaborates with the members of the interdisciplinary care team; prepares written documentation pertaining to patient evaluation and treatment sessions; schedules patient treatments and meetings; supervises the work of Rehabilitation Aides and Student Speech-Language Pathologists; assists in preparing and conducting inservice training and continuing education programs; and may participate in Performance Improvement activities and processes.Note: Position will provide services to Paola Hospital, Olathe Hospital, Paola Outpatient clinic, and Olathe Medical Pavillion D Outpatient Clinic.Responsibilities and Essential Job Functions
- Conducts Speech-Language Pathology evaluation in accordance with education, training, professional codes of ethics and standards, and department policy and procedure. - Educates nursing personnel, patients, and families, and professional staff, including Occupational Therapy, Speech-Language Pathology, Social Service, and Nutritional Care staff as to the objectives of Speech Language Pathology. Enlists the aid of other professionals in observing patient's speech and swallowing patterns and carrying out suggestions made by the SLP. Informs patient and family of problems, goals, estimated completion date, and treatment plan. Instructs patient and family in home management procedures.

- Completes and hands in end user audit reports and volume reports on the date of service and completes accurate charging of patient based on services provided, number of evaluations, screens, and hours in meetings, and number of visits, units billed, and cancellations.
- Supervises work of Rehabilitation Aides, through orienting, instructing, and directing work activities of Rehabilitation Aides as pertains to their assistance in patient treatment programs and department operations."

Major Responsibilities:

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
8. In some roles, this position may supervise staff and work flow of the department.

MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

This position involves daily reactive and targeted proactive medication regimen reviews for patients within the designated area(s) of responsibility or populations served. The Pharmacy Clinical Specialist position supports the delivery of pharmacy managed protocols, formulary processes and medication reconciliation activities while providing education to fellow pharmacists, nurses, and medical staff. Additional duties for this position include precepting pharmacy students and pharmacy residents on clinical patient care rotations within areas of responsibility and contributing to fulfilling the organization's mission to provide innovative, personalized, cost-effective pharmaceutical care in a culture dedicated to quality and safety.

Reporting to the Prior Authorization Supervisor, the Clinical Authorization Specialist is responsible for managing all work related to molecular pathology, high-cost drug, and off-label drug authorizations. The Clinical Authorization Specialist is also responsible for managing denials related to front-end prior authorization, biosimilar drugs, pharmacy-benefit exclusion drugs, and claims.

With direct oversight, the Clinical Authorization Specialist:
- Manages approval process for clinically complex cases:
- Discusses complex medical necessity cases in all aspects of the prior authorization work (on-label drug, off-label drug, laboratory testing, and others as assigned) with attending physicians.
- Understands clinically complex medical situations and communicates appropriate medical information to the insurer.
- Completes medical literature searches and/or coordinates appropriate provider-to-payer medical director discussions.
- Collaborates with attending physicians on treatment alternatives when medical necessity coverage denials cannot be overturned.
- Appropriately escalates complex cases to Drug Authorization Supervisor or Manager of Prior Authorizations.
- Monitors email communication to the Drug Authorization Mailbox and distribution lists:

- Creates cost estimates for waivers.
