Schedule
(2) 12-hr shifts per week: 6:45PM- 7:15AM; rotating weekends
Education
Preferred - High School diploma or equivalent
Work Experience
Required - None.
Preferred - 1 year of Patient Care Assistant/Nursing Assistant work experience in acute care hospital, long-term acute care hospital, or equivalent.
6 months of therapy experience, e.g. PT/OT Rehab Technician.
Nursing student who has completed first clinical rotation. Pursuit of professional medical licensure in any field applicable to an acute care setting.
Certifications
Required - Basic Life Support (BLS) from the American Heart Association
Knowledge Skills and Abilities (KSAs)
- Ability to function as a team member, responding willingly and positively to colleagues' needs for assistance and partnership in patient motility.
- Ability to promote a positive work environment with the intraprofessional care team.
- Ability to problem solve, assists other staff, transporters, and patient care technicians with mobility, while managing several ongoing priorities with minimal supervision.
- Proficiency in using computers, software, and web-based applications.
- Demonstrate complete understanding of Progressive Mobility Protocol (PMP) as well as documenting in EPIC.
- Effective verbal and written communication skills and ability to present information clearly and professionally.
- Strong interpersonal skills.
- Organizational skills.
- Ability to follow infection control protocols, safety protocols, obtaining vital signs, documenting in the medical record, inspecting, and maintaining equipment.
Job Duties
- Function as a team member and respond to colleagues' needs for assistance with patient mobility. Work collaboratively with nursing staff, therapy, and physicians to aid patients with mobilization needs.
- Assist with delegated treatment plan activities that include patient mobility, transporting patients, assisting patients in and out of bed, lifts, and ambulation.
- Gather patient information prior to treatment sessions according to the Progressive Mobility Protocol (PMP) and document in EPIC.
- Assist with equipment inspection, maintenance, and storage, cleaning/disinfection, and supply procurement.
- Collaborate and communicate with the interprofessional team regarding patients' response to delegated activities. Maintain accurate and complete documentation of patient care procedures performed.
- Identify opportunities for process improvement and provide recommendations impacting quality and safety targets on the unit-based scorecard, through peer-to-peer accountability, reporting near misses, and collaborating with the interprofessional team. Contribute to meeting department and hospital targets for quality and safety.
- Assist in gathering and maintaining data for quality improvement projects, inventory of supplies in the department, and prioritize daily tasks.

Very Heavy Work - Exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Heavy Work. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. Duties performed routinely require exposure to blood, body fluid and tissue.
The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.

POSITION PURPOSE:
At the direction of the radiation oncologist, and in tandem with a certified Radiation Therapist, assists in the radiation treatment process according to established guidelines and polices/procedures.
What the Radiation Therapist will do:
Assists in the review of a patient’s treatment record for completeness.
Confers with physicians, physics/dosimetry, and their colleagues for clarification prior to, during, and after treatment when necessary.
Supports the review of the electronic chart (Aria) and patient treatment record for consistency, completeness, and accuracy prior to treatment delivery.
Prepares treatment room and equipment according to established procedures. Explains procedures to patients according to departmental standards.
Follows standards in positioning of patients and follows treatment processes according to established policies and procedures.
Monitors patient during procedures at all times and reports any change in condition, psychologic, or physiologic to the physician or other appropriate patient care personnel.
Assists in the simulation and treatment planning process and other procedures occurring within the department of radiation oncology when necessary.
Acquires imaging via CBCT or portal imaging as prescribed by the physician.
Appropriately and thoroughly reviews images prior to treatment, ensuring alignment falls within allowable tolerances. Makes shifts and/or realigns patients when necessary to ensure the most accurate treatment delivery
Schedules patients for treatment and simulations per department guidelines in the Aria scheduling system.
Inspects all equipment safety interlocks, monitors equipment, and reports malfunctions.
Submits charges into the computer systems for services rendered to patients in accordance with established guidelines.
Participates in the clinical education and training of students.
Adheres to ALARA guidelines during all procedures.
Behaves in accordance with the Mission, Vision and Values of Trinity Health.
Other duties as assigned.
What the Radiation Therapist will need:
Education: A Bachelor's Degree in Radiation Therapy or completion of a two (2) year program in Radiologic Technology followed by a one (1) year program in Radiation Therapy.
Experience: Minimum of 3 months of on the job training or experience.
Certification: CPR. ARRT (American Registry for Radiologic Technologists) for radiation therapy. Must be registered or registry-eligible upon hire. Registry-eligible candidates must successfully complete registration within 6 months of hire.

Transitions of care pharmacists are responsible for target interdisciplinary programs and clinical services and ensure pharmaceutical care programs are appropriately integrated throughout BH system.
Participate in all necessary aspects of the medication-use system while supporting comprehensive and individualized pharmaceutical care in their assigned areas.
Serve as clinical resources and liaisons to other departments, hospital personnel, or external groups. In their role, they may conduct practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
Works collaboratively with physicians and other members of the healthcare team

What you’ll bring.
- An active Physician Assistant (PA) or Nurse Practitioner (NP) License by the MT State Board.
- Prescriptive Authority by the MT state board of Nursing or Medical Examiners and DEA License.
- Current BLS certification endorsed by the American Heart Association.
- ACLS Certification within 6 months of employment (site specific).
- Ability to communicate effectively and diplomatically within a multi-functional team.

POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. 1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. 2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.

The Speech-Language Pathologist consults with, advises, reports to and is responsible to the Manager of Physical Medicine and Rehabilitation.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Provides evaluation and diagnosis of speech, language, swallowing and cognitive-communication disorders.
Designs and implements individual treatment plans.
Counsels’ patients and their families concerning social, behavioral and psychological problems associated with communication and swallowing disorders.
Provides thorough documentation of test data and Speech-Language progress.
Provides patient and family recommendations and referrals to appropriate health, educational and vocational sources.
Participates in patient conferences when appropriate and communicates with individual members of medical staff concerning patient treatment as necessary.

Provides direction for graduate student affiliates and volunteers.
Maintain knowledge of current evidenced based practices in the field by reading the appropriate literature and attending professional conferences.

WORKING CONDITIONS

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

Requires Master's degree in Speech-Language Pathology.

State of Michigan Speech-Language Pathologist License or Speech-Language Pathologist Educational Limited Temporary License during their clinical fellowship year (CF), or eligible to obtain licensure prior to hire date. Employee is expected to attain the full license once within one month of completing Clinical Fellowship Year as defined as 36 weeks and 1260 hours.
In the case of a CFY, attainment of the Certificate of Clinical Competence (CCC) in Speech-Language Pathology through ASHA is required once 36 hours and 1260 hours of employment is completed.
Preferred:
Hold and maintain the Certificate of Clinical Competence (CCC) in Speech-Language Pathology through ASHA or be provisionally accepted during their clinical fellowship year (CF). To act as a supervisor for a student or CFY, CCC must be maintained.
American Heart Association BLS or equivalent
REQUIRED SKILLS AND ABILITIES
Analytical skills necessary to select and administer appropriate Speech-Language tests, differentially diagnose Speech-Language disorders, develop a treatment plan and modify treatment as patient progresses.
Strong written, verbal and interpersonal skills necessary to communicate effectively with patients, families, physicians and other professional/non-professional staff.
Ability to concentrate and pay close attention to detail for up to 70% of work-time when implementing evaluation/treatment services and maintaining patient documentation.

RESPONSIBILITIES:
Evaluates patients, establishes treatment goals/outcomes, and implements patient care plans. Observes precautions, indications, and contraindications for all treatment procedures. Monitors progress of patient, re-establishes goals, and revises treatment plans, according to the Rehabilitation Department protocol.
Provides written and verbal communication with physicians, nurses, social workers, and other members of the interdisciplinary team involved with the patient, on the patient rehabilitation status and recommendations.
Utilizes LifeChart to document significant clinical findings in the patient's medical record, according the department and hospital policy. Maintains confidentiality of patient information.
Effectively uses conventional art psychotherapy techniques such as art therapy assessment tools, CBT techniques, and trauma informed art therapy techniques to meet the goals and objectives of assigned patients. Provides a safe environment which is conducive to learning while adhering to department policies, procedures, objectives, quality assurance program, and infection control.
Provides Art Therapy supervision to undergraduate and graduate students. Communicates routinely with Senior Art Therapist?LMHC II to monitor student progress and compliance with individualized curriculums and clinical requirements.
Therapist must demonstrate competency in therapeutic process, as determined, by the clinical specialist, supervisor, or manager.
Participates in mandatory departmental, hospital-wide in-services, as well as obtain CEU's through external conferences to meet the standards and regulations of the AATA and NBCC credentialing boards.
Provides education and training to patients, patient caregivers, and families to effectively maintain levels of progress achieved in therapy program. Educates other health care professionals to the role of art therapy/mental health counseling, as well as providing training and in-services in areas of expertise.
Reviews and reassesses program materials, practices, activities, and clinical requirements making recommendations to the Senior Art Therapist/LMHC II or to the Rehabilitation Clinical Supervisor for modifications. Maintains and orders program materials and supplies.

Position requires acute care weekend rotation at the Aurora Medical Center - Washington County (Hartford, WI), approximately every 6 weeks and one holiday per year. Acute care shifts average 2-3 patient care hours each day but paid for a minimum of 4 hours each shift, regardless of whether actual work time is less than 4 hours. Acute care pay also includes an additional weekend/holiday premium added to hourly pay rate.

Pay Range:
$41.10 - $61.65Major Responsibilities:- Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
- Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
- Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
- Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, and ordering equipment.
- Completes all required documentation following department policy, including patient progress notes, team conference reports, and discharge summaries as appropriate, to record patient care and progress of each patient assigned as well as daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits.
- Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise.
- Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
- Pursues professional growth through participation in continuing education courses, department inservices, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
