ESSENTIAL FUNCTIONS
PEOPLE ESSENTIAL FUNCTIONS
- Contributes to teamwork, consistently offering assistance, and responding positively to requests for assistance. Collaborates to foster healthy relationships in the work environment. Offers guidance to interprofessional health care team, when appropriate, and applies feedback to improve patient outcomes.
- Uses therapeutic communication, which includes active listening and teaching, to establish a relationship with patients, families and interprofessional health care team to collaborate on the plan of care. Partners with members of the interprofessional care team by actively communicating and reporting pertinent patient care information and data in a comprehensive manner.
- Teaches and provides guidance to nursing, and support staff. Orients new team members and assists with precepting students in clinical rotations and clinical fellows. Uses peer-to-peer accountability towards improvement of department score for employee engagement.
SERVICE ESSENTIAL FUNCTIONS
- Performs complete and comprehensive evaluation of patients across a broad range of diagnosis and acuities to identify patient’s level of function and to determine the treatment plan. Develops the plan of care based on evaluation findings and implements the physical therapy treatment plan. Provides service to complex patient populations.
- Provides education to patients, patient’s family members, and/or caregivers, based on learning needs. Consistently evaluates the patient’s comprehension and adapts teaching methods accordingly. Serves as resource to clinical fellows and nursing in teaching best practices.
- Collaborates and communicates with the healthcare team modifications to patient’s treatment plan, progress, and discharge planning for achievement of identified patient specific goals. Advocates on patient/family’s behalf to identify and resolve clinical and ethical concerns, utilizing appropriate resources.
- Balances and adjusts as needed the caseload for the assigned team members to result in optimum patient care and outcome. Initiates improvements for department score on patient satisfaction, through peer-to-peer accountability to service standards.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Assesses patients regularly, monitors progress towards treatment goals, modifies goals, and plan or program as indicated. Recognizes complications/limitations associated with certain procedures and proactively addresses such for optimum outcomes.
- Provides select oversight of physical therapy-related activities provided by interprofessional health care team, and if necessary, adjusts the plan to improve quality of outcome and maintain the safety of the patient.
- Proactively identifies opportunities for corrective action and assists in process improvement by providing recommendations and contributes to meeting department and hospital targets for quality and safety. Identifies areas of practice improvements with research-based evidence and assists in development of treatment protocols and best practice for certain population or practice area to achieve patient safety outcomes, supported by accurate documentation.
FINANCE ESSENTIAL FUNCTIONS
- Maintains accurate and complete documentation of patient education and procedures performed in a timely manner in the patient record. Enters accurate charges for services provided, as applicable.
- Uses resources efficiently to maximize utilization of time and equipment. Self-motivated to independently manage time effectively, minimizing incidental overtime, and prioritize daily tasks, assisting coworkers as needed. Meets labor productivity standards by effectively managing established patient load assignments
GROWTH/INNOVATION ESSENTIAL FUNCTIONS
- Initiates innovative solutions through evidence-based practice/performance improvement projects. Actively participates in and may lead department projects and/or hospital committees. Participates in HM-related and approved hospital/community teaching opportunities, community services and/or functions.
- Identifies and presents areas for innovation, efficiency and improvement in patient care or unit operations using evidence-based practice literature. Assists higher level PTs with program development and/or clinical competencies.
- Seeks opportunities to identify self-development needs and expand learning beyond baseline competencies with a focus on continual development as a Physical Therapist, taking appropriate action. Ensures own career discussions occur with appropriate management. Completes and updates the My Development Plan on an on-going basis

Education Qualifications
- Clinical doctorate is required.
- Competence in delivery of healthcare services for adolescents, adults and geriatric patients is required.
- Licensed physician in the state of practice is required.
- Basic Life Support (BLS) is required.
- Maintain a Controlled Substance license for the state and Federal DEA, when applicable to their specialty.

Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care. Educates patient and family regarding treatment modalities of disease, which may include chemotherapy and radiation therapy and symptom management. Reviews patient’s clinical status and laboratory values pre-treatment and consults with provider when appropriate. Evaluates and determines need for patient referrals to other members of the interdisciplinary needs as appropriate. Identifies appropriate community resources to promote continuity of care and makes appropriate referrals. Manages patient care across the continuum, making appropriate referrals to home care and hospice, as necessary. Prepare and administers chemotherapy/biotherapy and supportive therapy in accordance with ASCO/ONS Guidelines. Recognizes and responds to emergency situations appropriately. Maintains BLS certification. Provides telephone triage according to policies and documents telephone interactions. Performs the following technical skills; Intravenous therapy including peripheral and central venous vascular access devices, including obtaining IV access, Safe handling practices for hazardous, cytotoxic and biological agents, Chemotherapy administration skills, including vesicant and non-vesicant medications. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role. Supports the DFCI Nursing Department Professional Practice Model and Quality Plan. Accurately records charges for all services rendered on the day service. Collaborates with Clinic Assistant to assure accurate inventory of all supplies. Serves as a resource to medical personnel in other health facilities, providing telephone consultation to engender optimal Oncology practices and knowledge among health care professionals caring for cancer patients.

Develops approaches to problem solving which meet the individual needs of the patients and families. MINIMUM JOB QUALIFICATIONS:
The position requires licensure as a Registered Nurse in the Commonwealth of Massachusetts or New Hampshire, as appropriate. - Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).

- Provides basic patient education on the performance of tasks, skills, treatment and self-care. - Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.

POSITION PURPOSE:
At the direction of the radiation oncologist, and in tandem with a certified Radiation Therapist, assists in the radiation treatment process according to established guidelines and polices/procedures.

Education: A Bachelor's Degree in Radiation Therapy or completion of a two (2) year program in Radiologic Technology followed by a one (1) year program in Radiation Therapy.
Experience: Minimum of 3 months of on the job training or experience.
Certification: CPR. ARRT (American Registry for Radiologic Technologists) for radiation therapy. Must be registered or registry-eligible upon hire. Registry-eligible candidates must successfully complete registration within 6 months of hire.
What the Radiation Therapist will do:
Assists in the review of a patient’s treatment record for completeness.
Confers with physicians, physics/dosimetry, and their colleagues for clarification prior to, during, and after treatment when necessary.
Supports the review of the electronic chart (Aria) and patient treatment record for consistency, completeness, and accuracy prior to treatment delivery.
Prepares treatment room and equipment according to established procedures. Explains procedures to patients according to departmental standards.

Position Purpose:
The Outpatient Infusion Visit Coordinator is responsible for the coordination of all chemotherapy and non-chemotherapy visits within the Cancer Center. This includes the accurate and timely scheduling of various types of therapies according to established protocols while considering the resources available (e.g., clinical team, pharmaceutical supply, infusion chair space, etc.). They will work with patients, providers, and the clinical team to ensure complete follow-through from the initial visit-to-final treatment.

Required Qualifications:
Education:
High school diploma or GED. Associate's degree or certificate in healthcare related field preferred.
Experience:
Minimum of one year scheduling experience in a health care setting. Preferred experience would include coordination of patient care in a complex care setting, cancer center, specialty practice, or other oncology setting; working with providers and caregivers and use of complex scheduling software.
Proficient in the use of an EMR software, EPIC preferred.
Working knowledge with Microsoft Office applications.
Basic understanding of medical terminology and reading physician orders.
Physical and Mental Requirements and Working Conditions:
Ability to exercise independent judgment to set and prioritize cases and defined workload.
Must be able to communicate effectively with internal and external providers.

What you will do:
Research individualized therapy plans (chemotherapy, immunotherapy, etc.) ensuring patients are scheduled according to established protocol and clinical best practices.
Can effectively coordinate the scheduling of a complex patient population within the constraints of clinical team availability, adequate pharmaceutical drug supply and/or blood products, and therapy chair space.
Actively engages with the onsite Lead Infusion Nurse Coordinator throughout the day and ongoing, to ensure adequate support of the infusion clinic (e.g., communicating that a new CADD pump treatment is beginning, or ensuring a cancellation is adequately managed).
Read and interpret physician office notes to ensure ongoing coordination of extended treatment plans requiring multiple visits over the course of several weeks or months.
Coordination of special procedures with our oncology providers including bone marrow biopsies.
Ensures coordination of visits adhere to Cancer Research study protocols, where applicable.
Coordinates patient schedules in collaboration with external departments including radiation oncology, urology, neurology, radiology, etc. Coordinates products from local Blood Bank for blood/platelet transfusions.
Can effectively prioritize stat/urgent requests to ensure escalation occurs in timely manner during critical patient situation (e.g., hydration, blood transfusions, and urgent chemotherapies).
Understands protocol and can effectively educate a patient on their required lab work prior to each relevant therapy visit.
Responsible for creating a hospital account (HAR), assigning referrals from work queue to appointments.
Responds to In-basket communications, secure chats, Teams messages, emails, and physician orders (Intent-to-Treat) in EPIC in a timely manner. Uses proper medical terminology to translate clinical information to schedule patients appropriately for treatment. Triages all incoming calls, assesses callers' needs, directs to appropriate department.

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.
- Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

We’re Looking For:
- Completion of an accredited or approved program in nursing
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
What You’ll Do:
- As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
- Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Physical Requirements & Working Conditions
Requires frequent sitting, standing, walking, bending, squatting, reaching, twisting, and lifting throughout the workday. Must safely lift up to 35 pounds and perform patient handling tasks, including pushing/pulling up to 30 pounds and assisting with 150-pound sliding transfers. Requires functional speech, vision, hearing, touch, and fine motor skills for clinical procedures, as well as the ability to operate equipment foot controls. Must be able to respond quickly to changing patient conditions and perform patient safety interventions, including verbal de-escalation and restraints when necessary. Exposure to blood, body fluids, communicable diseases, chemicals, and hazardous waste may occur; appropriate protective equipment must be used.
