$41.10 - $61.65Major Responsibilities:
Interprets physician referrals, as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods.
Establishes a plan of care and implements individual treatment based on evaluation results and functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs. Â
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning, transferring and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.  Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities and exercise. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care.Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.Â
May supervise physical therapy assistants following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. May supervise physical therapy extenders (rehab aides/techs) in administering treatments to patients and will teach them appropriate techniques to perform with specific patients. May supervise physical therapy students after completing one year of clinical experience.
Minimum Job Requirements
Education
Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation. Maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License
Licensed Physical Therapist in the State upon which you work. Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)

Position Summary:
The physical therapist assistant assumes the responsibility and accountability for providing physical therapy modalities and treatment procedures as determined by the plan of care established by a physical therapist and in accordance with the State of Michigan Public Health Code Act 368 of 1978, in addition to established facility and departmental policies and procedures. 2. Courteously greets patients.
3. Completes admitting information in computer.
4. Locates and assigns beds.
5. Notifies referring physician of admission, as requested.
6. Responsible for proper and expedient communication of transfer information when patient moved from one location in the hospital to another.
7. When transferring a patient within the same patient care area; completes transfer function in the computer.
8. When transferring a patient to a different patient care area, notifies the receiving department, obtains bed assignment, dispatches support associate
9. Responsible for clerical discharge functions.
10. Disassembles patient record once discharge order received.Â
11. Schedules all follow-up appointments.
12. Arranges transportation for patient as necessary.
13. Completes discharge function in the computer.
14. Faxes copy of discharge summary to designated areas
15. Coordinates and facilitates ongoing communication to ensure efficient intradepartmental and interdepartmental operations.

25. Completes laboratory requisitions and other forms accurately and according to departmental procedure.
26. Phones in consults to other services in a timely and accurate manner.

Core Clinical Responsibilities
Perform comprehensive device interrogations and troubleshooting for pacemakers, ICDs, CRT devices, and implantable loop recorders in inpatient, outpatient, OR, and procedural settings.
Independently reprogram cardiac devices within scope and in collaboration with Electrophysiology (EP) physicians and Advanced Practice Providers.
Monitor and assess device function during MRI procedures, applying appropriate MRI‑conditional programming, observing patients throughout the scan, and restoring baseline programming afterward.
Provide direct patient care by assessing device performance, identifying abnormalities, and initiating appropriate interventions or escalations.
Conduct and interpret remote device interrogations for ED patients and other hospital departments, ensuring timely response to alerts and transmissions.
Interdisciplinary Collaboration & Workflow Management
Collaborate closely with EP physicians, nurse practitioners, cardiologists, anesthesiology teams, and surgical staff to support safe and efficient patient care.
Share on‑call responsibilities with team members for urgent device support, including CVOR cases and emergent reprogramming needs specific to CVOR procedures.
Coordinate device‑related workflow across hospital units, managing device work queues and supporting care for hospitalized patients with implanted cardiac devices.
Participate in structural heart procedures (e.g., TAVR) by providing device‑related support such as burst pacing or procedural reprogramming.
Technical Expertise & Device Management
Demonstrate proficiency in operating and navigating multiple cardiac device manufacturers’ programmers, systems, and remote monitoring platforms.
Evaluate device algorithms, diagnostic trends, and therapy delivery to inform clinical decision‑making.
Troubleshoot device malfunctions, perform detailed assessments, and communicate findings clearly to EP providers.
Apply specialized device programming adjustments to meet individual patient needs, including comfort settings and modifications for hospice or end‑of‑life care.
Documentation & Compliance
Accurately document all device interrogations, programming changes, diagnostics, and clinical assessments in the electronic medical record (Epic).
Complete MRI safety screenings for patients with implanted devices according to current protocols and institutional guidelines.
Ensure all device‑related care follows established clinical pathways, manufacturer guidance, and regulatory requirements.
Patient & Family Education
Educate patients and caregivers on device functionality, safety considerations (e.g., electromagnetic interference), follow‑up expectations, and long‑term care of implanted cardiac devices.

Job Description:
The Advanced Practice Registered Nurse (APRN) is responsible for providing advanced practice care to patients with heart failure and other complex cardiovascular conditions. Â This includes conducting comprehensive patient evaluations, developing and managing treatment plans, ordering and interpreting diagnostic testing, prescribing medications, and providing ongoing disease management. Â Care is delivered in a Hospital Outpatient Department (HOPD). Â The APRN exercises clinical autonomy in medical decision-making within the scope of the Kentucky Board of Medical Licensure (KBML) collaborative agreement and organizational policies. Â He or she is a proficient user of all information systems involved in patient care, provides exceptional customer service to patients and colleagues, and maintains the highest standards of professionalism.
The individual may perform other duties, both major and minor, which are not listed below, and responsibilities may evolve over time based on the needs of the practice.
You will provide direct patient care to individuals with acute and chronic heart failure, cardiomyopathies, and other advanced cardiovascular conditions. Â Responsibilities include evaluating patient symptoms, performing comprehensive physical examinations, ordering and interpreting diagnostic studies, optimizing guideline-directed medical therapy (GDMT), monitoring response to treatment, and developing individualized care plans. Â You will collaborate closely with heart failure cardiologists, electrophysiologists, cardiothoracic surgeons, primary care providers, and other multidisciplinary team members to coordinate comprehensive patient care across the continuum. This role also includes patient education, medication management, coordination of outpatient and inpatient transitions of care, and helping reduce heart failure readmissions through proactive disease management. Participation in outreach, patient education initiatives, and occasional inpatient or weekend coverage may be required.
Duties and Responsibilities:
- Facilitate new patient consultations and follow-up visits for patients with heart failure and advanced cardiovascular disease.
- Perform comprehensive history and physical examinations.
- Order, review, and interpret laboratory studies, imaging, electrocardiograms, echocardiograms, and other diagnostic testing as appropriate.
- Initiate, titrate, and optimize guideline-directed medical therapy (GDMT) for heart failure in accordance with current evidence-based guidelines.
- Monitor patients for disease progression, medication effectiveness, and adverse effects.
- Coordinate care with cardiologists, primary care providers, electrophysiology, cardiac surgery, pharmacy, home health, and other specialists.
- Educate patients and families regarding heart failure diagnosis, medications, diet, sodium and fluid restrictions, weight monitoring, symptom recognition, and self-management strategies.
- Coordinate transitions of care following hospitalization to reduce readmissions and improve patient outcomes.
- Refer patients for advanced heart failure therapies, including mechanical circulatory support, structural heart evaluation, or heart transplant evaluation when appropriate.
- Participate in quality improvement initiatives focused on heart failure outcomes and patient education.
- Assist with inpatient consultation and weekend coverage as needed.

- Daily, provides ongoing Medical Nutrition Therapy (MNT) care for entire hospital inpatient census, through screening, prioritizing, assessing, and recommending interventions. Adapts daily workflow to accommodate fluctuations in patient load due to daily census variations, consults, acuity, staffing challenges. Develops and implements a plan of care for patients based on an expert assessment of life cycle- and diagnosis-specific nutritional needs. Assessment includes reviews of medical history and laboratory data, patient interviews, and physical examinations to determine the current nutritional status of inpatients and/or outpatients. Based on the assessment, the Clinical Dietitian develops and implements individualized nutrition plan of care interventions. Documents medical nutrition therapy plan of care in the electronic medical record supporting system clinical documentation and coding standards to maximize reimbursement rates.
- Prescribes enteral and parenteral nutrition support orders by assessing the patient and calculating macronutrients and/or micronutrients, fluid, and electrolytes. Evaluates patient tolerance and effectiveness of formulas and manages initiation or modifications to enteral/parenteral regimen orders. The Clinical Dietitian demonstrates extensive knowledge of specialized nutrition support (parenteral and enteral) and triages issues and concerns through collaboration with physicians and pharmacy. Constant, secure communication with system level Pharmacy Integrated Clinical Services (PICS) team to manage and order Total Parental Nutrition (TPN) infusions for critically ill patients in the hospital.
- The Clinical Dietitian prescribes orders for therapeutic diets, oral nutrition supplements, and nutrition support regimens, including vitamins/minerals, fluid management, and lab work as indicated within the scope of the Medical Nutrition Therapy (MNT) protocol. Utilizes order-writing privileges to implement nutrition related orders. Collaborates with Food and Nutrition patient services team to ensure patient intake aligns with nutrition prescription and interdisciplinary plan of care. Partners with site patient services team to identify and recommend appropriate clinical nutrition formula and supplement substitutions during supply chain disruptions to reduce and eliminate significant disruptions to patient care. Partners with physicians, nursing, and nutrition patient services to ensure diet restrictions, allergy restrictions, supplement preferences and tube feedings are consistent with patient needs and personal, religious, and cultural preferences. Develops patient-specific menus for patients admitted with complex dietary needs (e.g., multiple food allergies, numerous diet restrictions, and/or low-use, high risk diet orders).
- Leads efforts to identify, treat and prevent malnutrition. Performs Nutrition Focused Physical Examinations of high-risk patients to evaluate the loss of specific muscles and subcutaneous body fat to determine the presence of and degree of malnutrition. Communicates findings to the medical team through medical record documentation to ensure maximized reimbursement. Provides medical nutrition therapy education to patients identified with moderate or severe malnutrition to improve patient outcomes and to positively impact inpatient readmission rates.
- Routinely documents elements of the Global Malnutrition Composite Score (GMCS) for Medicare and Medicaid Services’ (CMS) Nutrition Electronic Quality Indicators. Participates in Quality Assessment Performance Improvement (QAPI) efforts to ensure nutrition care is safe and effective for patients.  Maintains regulatory readiness. Supports department clinical initiatives to ensure compliance with safety initiatives, policies, and procedures, DNV regulations, and department standards.
- Identifies and documents billable nutrition diagnosis with supporting documentation. May provide clinical support to various hospital-based outpatient medical destination programs (e.g., cancer center, transplant clinic, digestive health programs, bariatric, cardiac rehab, heart failure, etc.) Enters charges for outpatient nutrition charge capture in electronic medical record.
- Supports partnerships with local didactic programs by precepting dietetic graduate students in clinical nutrition and/or foodservice management. Directs and monitors interns’ progress both daily and at designated touchpoints and provides feedback for growth and development. Communicates regularly with didactic program coordinator. Also, precepts medical residents and provides nutrition orientation as applicable. Provides nutrition lectures to medical residents for internal medicine, trauma, NICU/Peds, and family practice, as applicable.
- Collaborates routinely with discharge planners, home health, physicians, pharmacy, and other interdisciplinary teams to address the full scope of the system-established priority points protocol for inpatient care. Participates in interdisciplinary team rounds, meetings, and care conferences to coordinate and collaborate on nutrition goals. Assists with transition to home or outside facilities by partnering with Care Management and/or Home Health to coordinate home enteral and/or parenteral nutrition needs.
- Provides medical nutrition therapy education to patients on prescribed diet, per established priority system, to improve patient knowledge/understanding of nutrition, diet therapy and compliance on prescribed diet. Education is provided to inpatients, outpatients and family members through individual counseling and classes. May provide nutrition in-services to teammates, as applicable.
- Provides clinical input into system clinical nutrition initiatives and practice changes. Participates in system subgroups/projects, as assigned, to develop and implement evidence-based nutrition practices, clinical initiatives and/or practice changes. Partners with system patient services and clinical teams to deploy system changes and updates regarding CBORD, EPIC, menus, emergency preparedness, downtime processes, formulary, clinical procedures, and policies. Liaisons between clinical nutrition and inpatient nursing and physicians to champion system clinical nutrition initiatives (e.g., new dysphagia standards, malnutrition coding practice changes, enteral nutrition equipment changes, malnutrition nurse screening changes, safety protocol changes, etc.) Assists (via in-person or remote coverage) other sites throughout the State, as needed, to provide clinical nutrition coverage to assure equitable patient care.

Minimum Qualifications
- Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.

The CV Invasive Specialist I performs duties necessary for cardiac catheterization procedures at a technical and professional level requiring some proctoring and supervision of technical detail. The Specialist I performs a variety of technical procedures that require some independent judgment. The CV Invasive Specialist I assumes responsibility and initiative for designated areas and procedures as required. Follows Alliance of Cardiovascular Professionals (ACVP) Scope of Practice or for Electrophysiology the NASPE Standards of Professional Practice for the Allied Professional in Pacing and Electrophysiology.
Essential Functions
- Participate in procedure room preparation and post procedure turn around.
- Develop specific knowledge with regard to invasive, diagnostic, and interventional supplies which includes assisting in inventory control. Develops specific knowledge of applications, proper use and deployment techniques including documenting hemodynamic recordings, calculations, and interpretations.
- Provide patient education and emotional support for patients and their family members; ensure patient comfort. Foster teamwork and cohesiveness with team members and physicians.
- Develop knowledge specific to cardiopulmonary pathophysiology, pharmacology, anatomic anomalies, and treatment options. Assess the condition of the patient, document, and inform physician of deviations from normal criteria throughout the procedure.
- Understand and apply principles of x-ray generation and safety. Understand and demonstrate the correct operation of imaging equipment.
- May mix and administer medications under the direction and supervision of the physician (RCIS or RCES is required to pass medications). Pursuant to state laws.
- At the end of the procedure, may apply closure device, suture the sheath in place, or remove the sheath and apply pressure to obtain hemostasis.
- Provide thorough and accurate documentation of each procedure and associated events.
- Develop skills in operating, calibrating, and troubleshooting equipment in the Cardiac Catheterization Laboratory. Perform point of care testing and QA.

MINIMUM JOB QUALIFICATIONS:
The position requires licensure as a Registered Nurse in the Commonwealth of Massachusetts or New Hampshire, as appropriate. A bachelor’s degree in nursing is strongly preferred, and candidates with an associate degree must complete a BSN within 4 years. Completion of the Oncology Nurse Residency Program (ONRP) or 1 year of recent experience as a Registered Nurse. Ambulatory and/or oncology experience preferred. Professional nursing certification (e.g., OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC) is preferred, and AHA BLS certification is required (AHA ACLS is acceptable as a minimum requirement).

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).

- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. REQUIREMENTS
- PharmD or BS Pharm with post 2006 experience.
- 1 year of pharmacy experience.
- BLS.
- Registered Pharmacist, licensed in good standing in Ohio and Michigan.
PREFERRED REQUIREMENTS
- 3 years of full time pharmacy practice or PGY1 Pharmacy Residency experience.
- Board Certification and/or additional training in specialty area.
