GENERAL SUMMARY:Provides the services to adults including: conducting face-to-face assessments, establishing diagnoses, making treatment suggestions; ordering behavior health intervention services (when indicated); and assisting with referrals to behavioral health intervention services providers (if requested); Provides substance abuse, co-occurring trauma, and personal growth-related therapy services to assigned clients and their families. Applies advanced psychosocial theory and methods to individuals as well as families and groups with appropriate regard for the complexity of given problems. Performs tasks and makes decisions that require independent judgment, are complex in nature, involve short and long range planning and require the use of high level problem solving techniques. Applies concepts of client- centered care to improve client outcomes. Coordinates internal/external services to provide as complete a continuum of care as possible.
ESSENTIAL FUNCTIONS:
- Establishes rapport and develops a therapeutic relationship with assigned clients.
- Provides therapy and counseling services in individual and group sessions. Maintains appropriate professional boundaries.
- Provides crisis intervention and conflict resolution assistance.
- Demonstrates ability to differentiate between emergency and non-emergency tasks and responds accordingly.
- Provides immediate supportive counseling and necessary assistance to clients dealing with a crisis or emergency situation.
- Assists with referrals to community resources when appropriate. Demonstrates the ability to perform in an accurate and precise manner in situations where time is critical.
- Conducts educational groups providing recovery-related information and instruction.
- Assists with referrals to community resources when appropriate.
- Provides counseling/treatment services to restore clients to optimum social and physical well-being (within their capacity). Provides counseling in individual and group sessions for co-occurring, trauma and personal growth-related concerns.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Bachelor’s degree in a human services related discipline from an accredited four-year program, required.
- Valid Driver’s/Chauffeurs License required, must meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age and be eligible to drive per Iowa state law.
- Proof of completion of Mandatory Reporter abuse training specific to population served within thirty (30) days of hire.
- Licensed to provide mental health therapy in the State of Iowa (LMHC, LISW, LMFT, etc.) and meets the requirements necessary for an LPHA designation OR
- CADC certification within one (1) year of hire.
- Ability to understand medical terminology and communicate effectively with healthcare professionals.

1. Provides coordinated and comprehensive patient care including assessment, expected outcomes, rehabilitation interventions and evaluation with an interdisciplinary team approach.
2. Evaluates speech, language, swallowing, and cognitive-linguistic needs and develops and implements the treatment programs for assigned patients to provide coordinated and comprehensive patient care including assessment, expected outcomes, and rehabilitation interventions within an interdisciplinary team approach. Determines/revises priorities for direct/indirect patient care activities based on patient need and available resources. Documents all treatments, according to department policies and procedures, as monitored by supervision, by documentation review, and consultation with referring Provider
3. Performs evaluation of patient response to tasks performed and delegated.
4. Participates in the Performance Improvement program of the Rehabilitation Services department with emphasis on speech therapy, to assure the delivery of appropriate, timely, and cost-effective services.
5. May supervise assigned graduate assistants/interns, students and volunteers. Assigns schedules for students to participate in evaluation and treatment of patients conducted in the clinical setting. Approves graduate student lesson plans, reports, documentation in hospital EMR, or appropriate written communication for assigned patients. Provides feedback to students during supervised sessions. Discusses techniques, goals, and materials to be used in therapy sessions. Provides written evaluation of student performance at the end of the semester.

1. Completion of annual required education related to specialty accreditation as defined by accreditation standards. Training may be completed through CBLs, trainings, In-services, and competency validation.

POSITION SUMMARY:
Provides physical therapy services under the supervision of a licensed physical therapist to patients across the age continuum. Assists the physical therapist within established guidelines to develop a rehabilitative plan of care with realistic goals, based on a functional evaluation or reassessment. SUMMARY
The Physical Therapist reports directly to the Clinical Manager and/or Assistant Clinical Manager (ACM) of their respective department and may receive clinical guidance from senior staff and/or clinical specialists.

Job Description:
Provides comprehensive therapeutic patient care to ensure positive patient outcomes. - Provides anesthesia services, including general, regional, and monitored anesthesia care to patients which includes: the induction of anesthesia; the maintenance of anesthesia at the required levels; the support of life functions during the administration of anesthesia, including induction and intubations; the recording of all pertinent events during the induction of, maintenance of and emergence from anesthesia; the recognition and the taking of appropriate corrective action (including requesting consultation when necessary) for abnormal patient responses to anesthesia or to any adjunctive medication or other form of therapy; and the professional observation and resuscitative care (including requesting consultation when necessary) until the patient has regained control of his/her vital functions.
- Performs pre-anesthesia functions including: determining that an appropriate pre-anesthetic examination has been made by a physician; insuring that relevant tests have been completed and a thorough assessment of the patient has been performed; reviewing the patient's condition immediately prior to the induction of an anesthesia; checking the readiness, availability, cleanliness, sterility when required and working conditions of all the equipment used in the administration of anesthetic agents prior to administering anesthesia.
- Performs post-anesthesia duties including: remaining with the patient until it is safe to transfer responsibility for care to other qualified individuals; advising personnel responsible for post-anesthetic care of specific problems presented by the patient's condition; administering post-operative epidural pain relief and other post-operative analgesia in collaboration with the surgeon and making post-anesthetic visits regarding the presence or absence of anesthesia related complications.
- Responsible for narcotic accounting and distribution.
- Responds to life threatening, Code Blue/Code 4 and respiratory emergencies if available by establishing and maintaining an airway under the direction of a physician. Assists nursing units with difficult intravenous (IV) insertions when requested, when available and when attempts by other professionals have been unsuccessful.
- May administers anesthesia under the supervision of the operating surgeon.
- May provides labor analgesia in the Birth Day Center.
- Maintains knowledge of scientific theory, research findings, current practice and applicable standards.

Major Responsibilities:
Supports the orientation process by developing, coordinating, facilitating, conducting and evaluating orientation programs for nursing and other healthcare personnel.

Designs educational activities to address practice gaps for identified target audiences in consultation with a Nursing Professional Development (NPD) Specialist.

Evaluates and analyzes the impact of programming on learners, organizational resources, and patient outcomes and makes adjustments in consultation with an NPD Specialist as needed.
Establishes and implements appropriate evaluation tools and methodologies to identify needs or gaps in the knowledge, skills, and competencies of nursing personnel to advance the practice of nursing in consultation with an NPD Specialist as needed.
Develops and supports education needs based on specialty or department in collaboration with regional and system teams.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Essential Functions

In addition to managing device clinic and remote monitoring for 6,000+ patients, we have established innovative collaboratives with Radiation Oncology, MRI, Proton Therapy, and CV imaging to provide the excellent care to our device patients. We host specialized clinics for advanced cardiac devices including CRT, left bundle branch pacing, and cardiac contractility modulation. We are a rapidly growing team with ambitious goals; we analyze remote transmissions within 24 hours. This is a wonderful opportunity for a new or experienced cardiac device nurse. The Cardiac Device RN is a licensed registered professional nurse who is a clinical expert in cardiac device management and provides ethical, evidence-based, safe, and compassionate nursing care. The Cardiac Device RN focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs related to cardiac device, conducting education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The Cardiac Device RN will assist with coordination of care amongst disciplines and departments including but not limited to General Cardiology, EP Cardiology, Advanced Heart Failure, Primary Care, Acute Inpatient Care, PeriOp, Oncology. The Cardiac Device RN will be responsible for providing comprehensive patient care by completing thorough, competent interrogations and reprogramming of implanted cardiac devices in the inpatient and outpatient settings. The Cardiac Device RN will assist in coordination of care and device optimization throughout imaging studies. The Cardiac Device RN is responsible for triaging remote transmissions and programming recommendations for patients undergoing therapies that may impact the device.Responsibilities and Essential Job Functions
- Coordinate care by triaging orders, device data and the clinical scenario to provide recommendations for programming of device patients undergoing proton therapy, radiation oncology, spine center, peri, post operative and/or MRI.
- Coordinate care for hospitalized device patients by performing in-person device evaluation, communicating to vendor representatives, or utilizing remote transmission technology.
- Telephonic assessment and guidance for cardiac device patients to provide seamless coordination of care utilizing device data to optimize patient health.

RESPONSIBILITIES:
Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.
Ensures pharmacy services in area of practice are provided in accordance with established policies and procedures and federal and state rules and regulations.
Receives patient medication orders or prescriptions from practitioners in the outpatient setting. iSpecific Retail and Specialty Pharmacy Responsibilities:
Reviews, approves, and enters medication orders in the computerized pharmacy information system including appropriate product formulation, dosage, frequency, and duration of therapy. Ensures relevant patient specific parameters are included on the patient’s medication profile (i.e., diagnosis, known allergies, weight, age, etc.). Reviews and approves all medication orders and patient specific parameter computer entries if delegated to technician staff.
Dispenses ordered medications in the appropriate quantity to ensure patient receives the correct medication, in the right dosage and dose form, at the proper time.
Reviews and approves formulations for medications requiring compounding or manipulation prior to dispensing. Checks the final product prior to dispensing medication, if preparation is delegated to technician staff, in accordance with established procedures.
Maintains appropriate documentation and retains records regarding drug purchasing, storage, preparation, and distribution in accordance with established policies and procedures and existing federal and state rules and regulations.
iAdditional Specific Specialty Pharmacy Responsibilities:
Adheres to the policies and procedures of the Specialty Drug Patient Management Program (SDPMP) and encourages patient participation in the Program.
Performs patient outreach and assessments in accordance with the SDPMP.
Participates in the development and execution of patient Plans of Care.
iSpecific Medication Access Program (MAP) Responsibilities:
Provides MAP refill authorization services to Brown University Health patients per existing collaborative practice agreements and protocols in accordance with established policies and procedures and existing federal and state rules and regulations.
Navigates the patient’s electronic health record (EHR) to investigate and answer questions related to MAP services. Completes and documents medication benefit investigations, prior authorizations, medication reconciliations, and/or medication financial assistance services. When appropriate, provides alternate therapeutic drug recommendations according to established guidelines.
Monitors accuracy and efficiency of MAP technicians refill processing, prior authorization submissions, denials and appeals management, and coordination of medication deliveries. Provides real-time feedback to improve case management and outcomes.
Participates in identifying operational trends and issues to help develop systems and workflows to improve productivity and efficiency and promote MAP success. Ensures MAP activities operate in accordance with established clinical standards of care and meet or exceed patient quality standards.
iSpecific Clinic-based Program Responsibilities:
Monitors and interprets laboratory results and adjusts medication regimens per existing protocol and collaborative practice agreements in accordance with established policies and procedures and existing federal and state rules and regulations.
Identifies out-of-scope patient care issues and triages them to appropriate member of the patient’s healthcare team.
MINIMUM QUALIFICATIONS:
BASIC KNOWLEDGE:
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
Prior to start date, possesses current unrestricted licensure as a licensed pharmacist, or as a licensed 90-day temporary pharmacist, in the State of Rhode Island. Obtains full licensure prior to expiration of 90-day temporary pharmacist license, then possesses and maintains current unrestricted licensure as a licensed pharmacist in the State of Rhode Island.

Where You Will Work:
4EF Medical/Telemetry – Respiratory Focus
4EF is a 24-bed, all-private room unit specializing in respiratory care and is the only non-ICU unit at Luke’s caring for ventilator patients. While respiratory is our focus, we also treat a wide range of acute and chronic conditions, including COPD, CHF, pneumonia, sepsis, diabetic ulcers, and cellulitis. Our patients are high-acuity—just below ICU level, making this an excellent environment for skill development.
What You’ll See:
- Ventilators (up to 4 chronic vent patients)
- Chest tubes, trachs, feeding tubes, ostomies
- Wound vacs, ports, suprapubic catheters
- IV therapy, drips, NG tubes, drains

We’re Looking For:
- Completion of an accredited or approved program in nursing
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association .
What You’ll Do:
- As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
- Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Assesses and diagnosis patients in a practice setting. Identifies outcomes, implements treatment and coordinates care. Performs and assists with oncology and infusion related treatments, therapies, and procedures.
Essential Functions
- Identifies expected outcomes for patients' individualized treatment plan.
- Implements identified treatment plan and coordinates all aspects of care delivery.
- Provides education and counseling to patient and family members regarding identified health needs, health care activities, immunization requirements, medication effects and contraindications.
- Meets specific clinical needs of oncology and infusion patients by using knowledge and expertise.
- Recognizes medication mid solution incompatibilities and factors affecting stability.
- Performs facility acuity coding by selecting the appropriate level of service for a diverse patient population, ensures that all pertinent clinical information is documented in the patient's medical record and supports the level of service selected.
Work requires walking, standing, sitting, lifting, reaching, bending, pushing and pulling. Work requires lifting 75 pounds. Must speak English fluently and in understandable terms. Must possess an intact sense of sight, hearing, smell, touch, and finger dexterity. Must have the ability to concentrate. Must be able to respond quickly to changes in patient and/or clinic conditions.
