Job Summary:
The PT evaluates the patient's comprehensive health care needs in the outpatient clinic and implements an appropriate plan according to those needs utilizing the physician, community resources and the patient's support system. Utilize critical thinking skills for the care of the patient. Provides skilled physical therapy procedures/assessments and instructs patients and their families in the plan of care. Utilize electronic records to document treatment & care.
Requirements:
- Current active or current temporary Indiana Physical Therapy License
- BLS certification within 90 days of hire
- Ability to work 40 hours per week with occasional late or early shifts to accommodate patients

PEOPLE ESSENTIAL FUNCTIONS
- Functions as a team member, responding willingly and positively to colleagues’ needs for assistance and partnership. Promotes a positive work environment with the interprofessional care team. Problem-solves, assists other techs and Patient Care Assistants, managing several ongoing priorities with minimal supervision.
- Works collaboratively with the department nursing staff and physicians, providing assistance to patients, based on previously identified needs, in practicing post-surgical or strengthening exercises or functional activities, utilizing safety measures at all times in regards to patient care.
- Initiates contributions towards improvement of department scores for turnover/retention/employee satisfaction on unit-based scorecard, i.e. peer-to-peer accountability.
SERVICE ESSENTIAL FUNCTIONS
- Follows the patient and family-centered care standards and provides direct patient care as appropriate. Assists with delegated treatment plan activities that include but are not limited to: patient mobility, transporting patients to and from treatment areas, assisting patients in and out of bed, assisting with lifts, ambulating patients, assisting patient for wound care treatments and for wound rounds.
- Gathers patient information prior to assign patients’ treatment session. Plans and organizes materials and supplies in the appropriate work areas prior to patient arrival, including but not limited to cleaning/disinfecting, gathering equipment. Assists with equipment inspection, maintenance and storage, and supply procurement, keeping hallways and room egress free of clutter.
- Assists with responding to patient falls, acts as a first line steward of service recovery, and assists and provides information related to re-enforcing safety awareness and home exercise programs for patients.
- Role models behaviors and skills, through peer-to-peer accountability, towards improving department score for patient satisfaction on unit-based scorecard, i.e. bedside shift handoff/report.
QUALITY/SAFETY ESSENTIAL FUNCTIONS
- Collaborates and communicates with interprofessional team the patients’ response to delegated activities. May, if requested, maintain accurate and complete documentation of patient care procedures performed. Provides for the privacy of patients and families, keeping the safety of the patient in mind.
- Supports initiatives to prevent conditions such as pressure injuries, patient falls, and hospital-acquired infections by following infection control processes and conducting hand hygiene audits.
- Identifies opportunities for process improvement providing recommendations impacting quality and safety targets on the unit-based scorecard, through peer-to-peer accountability, reporting near misses, and collaborating with the interprofessional team. Contributes to meeting department and hospital targets for quality and safety.
FINANCE ESSENTIAL FUNCTIONS
- Assists in gathering data for quality improvement projects, daily charge reconciliation for productivity and inventory of supplies in the department.
- Uses resources efficiently; does not waste supplies. Self-motivated to independently manage time effectively and prioritize daily tasks, minimizing incidental overtime. Meets labor productivity standards by effectively managing established daily responsibilities and assignments. Role models effective time management and coordinates colleagues to meet the overall demands of the unit.

- Seeks opportunities to expand learning beyond baseline competencies with a focus on continual development as a Mobility Technician. The Radiologic Technologist provides diagnostic imaging services using radiographic and fluoroscopic technology to support accurate diagnosis and treatment. This role performs imaging procedures, image processing, and related techniques to produce high-quality images for interpretation by a radiologist or licensed independent practitioner. The Radiologic Technologist applies professional judgment, upholds high ethical standards, and adapts to varying patient conditions to ensure safe, effective, and high-quality exams.
Key Responsibilities:
- Performs radiographic and fluoroscopic procedures utilizing x-ray technology to assist in diagnosis and treatment
- Conducts image acquisition, processing, and post-processing to produce high-quality diagnostic images

- Assesses and responds to varying patient conditions to ensure appropriate care and successful completion of exams and procedures

- Completion of an accredited program in Radiologic Technology

The Rehab Technician is a non- licensed caregiver who works under the direction of a Physical Therapist, Occupational Therapist, Speech Therapist, and/or Assistant to aid in the provision of patient care. Under the direction of Clinical Nurse Manager (Inpatient)/Program Director (Partial) and Unit Charge RN, the Behavioral Health Specialist I (BHS I) is responsible for providing care and safety to children and adolescents as a member of the interdisciplinary treatment team. RESPONSIBILITIES:
A. Participates in maintaining a safe and therapeutic environment. Actively engages with assigned children throughout shift. Participates in the development and operationalization of the milieu care plan for assigned patients according to established department and unit-based procedure/policy. Maintains and documents level of patient observation as ordered by the physician/charge RN each shift. Provides support to visiting parents as well as information according to treatment team request. C. Actively participates in the patient care planning process under the direction of the primary/charge RN. Works to support initial therapeutic alliance with patient and family on admission.Assists with the separation process and performs other duties necessary to provide a smooth transition to the hospital environment. Within the 8-hour shift, documents objective observations within a shift note and shares observations with assigned RN. Reviews milieu care plan and problem list on a daily basis and suggests revisions based on observations. Communicates individual patient milieu care plans in an ongoing manner to other unit staff including liaison with other interdisciplinary treatment providers. Actively participates in treatment team, rounds, patient-specific discussions, and hand-off shift report, as assigned. Accurately documents significant patient/family observations/milieu care on the Kardex and medical records in a timely fashion. Offers information and knowledge acquired through experience to other staff via formal/ informal staff development forums. Is able to adapt care “style” to meet needs of various children and adolescents.
D. Supports and contributes to the development of care plans/behavioral management. Assists and teaches patients in activities of daily living and personal hygiene based upon patient’s abilities. Co-leads community meeting/milieu meeting/social skill group. Co-leads other program specific groups such as activity or recreation. Demonstrates ability to set consistent and supportive therapeutic limits within guidelines of hospital, unit, and department standards. Follows behavioral programs specific to the needs of the patient in collaboration with the assigned RN and treatment team. Organizes unit activities. Coordinates, supervises and actively participates in children’s activities such as physical &social development activities. Arranges for transportation of materials if required. Participates in teaching visits/family and parent training as assigned. Consistently models to patient/families appropriate and therapeutic interpersonal staff/patient interactions. Co-leads therapy groups with a clinician. Supports and actively participates in recreation therapy programs. Handles crisis situations in a safe, effective manner, utilizing the least restrictive, safe patient care alternative. While an individual is hired to work on a particular program, they may at times be asked to perform comparable duties in other clinical settings throughout the hospital system as program needs arise.
BASIC KNOWLEDGE:
Baccalaureate Degree in a Human Service field preferred. Other education and experience considered and may be substituted for educational requirements at the discretion of the Hiring Manager.

Demonstrated knowledge and skills necessary to provide care to patients throughout the life span with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Safety Care Certification: Employees in this position work in a psychiatric setting under stressful conditions associated with care of psychiatrically impaired, aggressive, and sometimes, self-injurious individuals. Employees are expected to utilize de-escalation techniques taught by Bradley Hospital to minimize need for physical intervention. They must have the ability to remain calm during a crisis situation as evidenced by the ability to make quick rational decisions and prioritize immediate patient care needs to maintain a therapeutic environment and ensure safety. Employees in this position must also be able to participate in the physical management of patients which can include, but is not limited to, awareness and demonstration of safe body positioning at all times, awareness and coordination to block and deflect possible aggression, and the ability to physically hold struggling patients, using approved methods only, to maintain safety for extended periods of time. Specifically, such employees must be able to: lift up to fifty pounds; reach with their hands and arms; grasp and hold with their hands; bend/squat/kneel without limitation, and physically escort patients who may become combative for short distances. Essential Functions:
1. Evaluate individuals seeking behavioral health/substance use treatment; determine provisional diagnosis and discuss treatment recommendations with patients, families, and medical providers.
2. Coordinate LOC expectations with physicians, the Assessment Referral Center, patient care units, and external agencies; present clinical information to medical providers and collaborate with nursing on medical needs and appropriate level of care.
3. Conduct assessments remotely via telehealth and/or on-site at local hospitals and community sites; provide assessment services across pediatric, adolescent, adult, and geriatric populations as needed.
4. Perform insurance prior authorizations/pre-certifications and support intake for admitted patients.

- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).

This includes independent order writing privileges for managing nutritional care of our patients including parenteral and enteral nutrition. Job Duties:
Provides safe, ethical, and competent evidence-based nutritional therapy that meets the standards of practice and the scope of practice as stated by the Academy of Nutrition and Dietetics. Manages the individual’s nutrition care in collaboration with the health care team and other nutrition professionals.
- Using the Academy of Nutrition and Dietetics’ and Nutrition Care Process, assesses individual needs through synthesis of information obtained through the assessment interview, food and nutrition and related histories, review of the medical record, and discussion with other members of the interdisciplinary health care team in assigned area
- Develops a plan to meet assessed needs including recommendations regarding: the composition of diet and meal patterns and supplements; and the volume, rate, and composition of parenteral and enteral feedings.
- Confers with the health care team to develop goals related to patient care.
- Documentsin accordance with established standards.
- Evaluates nutritional status of individuals and makes recommendations regarding provision of optimal care.
- Counsels individuals, families, and care provider on normal nutrition and therapeutic diets with consideration given to the individual's educational, cultural, and socioeconomic background.
- Plans and conducts group classes for individuals in various areas of nutrition asindicated.
- Writes nutrition related orders for hospitalized patients in accordance with the Clinical Nutrition policy as indicated.
- Consults, educates, and advocates on behalf of individuals, groups, and special populations regarding food, culinary, nutrition and health issues, food security and insecurity, nutrition programs, nutrition wellness and resources.
- Plans for continuing nutrition care for individuals, as needed,using ancillary resources.
- Self-evaluates results and monitors for trends and changes in productivity related to workload.
- Compiles statistics and completes Clinical Productivity worksheets on a timely basis.
- Participates in departmental initiatives to meet strategic goals.
- Actively participates on appropriate committees for the organization and represents the Clinical Nutrition department in decisions made by these committees.
