Responsibilities
Complies with facility policies and procedures for appropriately documenting, billing and coding for residents receiving speech language pathology and audiology services.
Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speech language pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User’s Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master’s Degree, or equivalent, in Speech Language Pathology
A valid, unrestricted license in his/her therapy discipline in the state.
Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

- Assists with discharge services including preparing discharge packets, providing current resource lists, and researching resources to meet identified discharge needs.
- Contributes to efficient and effective Departmental workflow through assigned responsibilities including: arranging transportation; maintaining clothing closet; assisting with guardianship application process; assisting ED staff; supporting computer hardware and software needs; completing payroll; managing supply/delivery of car seats; appointment scheduling; delivering equipment to patient's room and completing quality assurance audits.
- Ensures the delivery of required Medicare forms, including Medicare Important Message (IM) letters to all Medicare and Medicare Advantage inpatients within 2 days of patient discharge, MOON forms to all Medicare and Medicare Advantage patients in Observation status for more than 24 hours, and Condition Code 44 letters to Medicare and Medicare Advantage patients when the patient status has been changed from inpatient to observation status.
- Facilitates smooth exchange of necessary information by communicating with Care Coordination staff, other medical center staff and outside agencies.
- Completes ad hoc reports, filing, data entry and general clerical duties as needed; prioritizes and organizes work to ensure timely submission of needed reports at the request of leadership.
- Performs reception activities by responding to questions, assisting customers, and answering the telephone in accordance with departmental standards. As appropriate, escalates callers/issues to Departmental leadership.
- Completes all required staff education by the assigned due date.
- Documents all work activities on the day on the day of service provision.
Education, Experience and Certifications
High School Graduate; Associate degree in Secretarial Science or health related field preferred. 2 years office experience preferred.

Position Purpose:

What You Will Do:

Summary:
Creates a sanitary, safe environment and enhances the appearance of the facility.
Responsibilities:
Maintains cart and equipment in clean, working condition and stores in appropriate storage area when finished with shift.Cleans patient rooms and other assigned areas daily, including collecting and removing trash, changing bed linens and sanitizing all surfaces. Ensures proper sanitation of medical equipment as required.May be responsible for maintaining a consistent floor care program according to outlined policies and procedures.Replenish supplies and/or linen in patient rooms as necessary or designated.Performs other environmental services duties as needed.
