Summary:
Performs administrative/clerical duties in support of an assigned nursing area.
Responsibilities:
Assists with processing patient admissions, transfers and discharges. Assembles and maintains patient records and charts.Performs needed clerical tasks, including copying, faxing, filing, and ordering/obtaining supplies.Greets patients and visitors. Monitors, answers, screens, and routes all telephone calls. Emergency alerts, pages, and overhead announcements as requested.

Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
May schedule walk-in appointments for services offered.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested. Prepares and balances a daily deposit of all payment collections.Explains polices including all regulatory and financial consent forms; secures all required signatures.May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Interviews patients and guarantors at the workstation or bedside to obtain all necessary information, including a copy of the patient or guarantor identification card.Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.May confirm physician and prescription orders ensuring accuracy.Obtains pre-certification and authorization.

Other Information: In Memorial Physician Group (specialty practices), additional responsibilities include: (1) obtain specialty authorizations (2) authorization denial and peer to peer process (3) patient care navigation ex: surgical and procedural coordination and scheduling for patient specific populations (4) handle all incoming calls and physician and hospital back line (5) obtain and confirm referralsIn the Hospital, additional responsibilities include: (1) Upon validation of patient identity, place identification band on patient (2) obtain signatures for hospital specific regulatory forms not required in an ambulatory or office setting (3) obtain authorizations for walk-in appointments (4) determine when financial assistance is needed.In Memorial Primary Care, additional responsibilities include: (1) MIH-MPC program patient referral, payment collection and eligibility scheduling (2) process referral work-ques and same day access requests (3) work with Patient Access Center on real time patient requests (4) address prescription refill requests, patient advice requests through MyChart, and provider scheduling template.

Summary:
Under the supervision of a RN, the Monitor Technician monitors, recognizes, and interprets cardiac rhythms while identifying deviations in patients' baseline, and changes in SP02. The Monitor Technician notifies the registered nurse of changes to a patient's status and initiates the appropriate emergency response by the critical care team in the event of a lethal arrhythmia, including irregular heart rhythms and cardiac arrest.
Responsibilities:
Records information in patient's log and maintains accurate, neat account of patient's cardiac monitoring status during episode of care. Identifies and records patient's rhythm and changes in patient rhythm and condition. Updates patient's records with all readings taken from the heart monitor or EKGs.Documents the electrocardiograph tracings of patients on telemetry monitors.Identifies arrhythmias, deviation in each patient's baseline, signs of patient deterioration, and intervenes to ensure patient safety.Reports abnormal or change in patient's telemetry monitoring to nursing staff or immediate supervisor or designated chain of command.Identifies and corrects for artifacts.Attaches electrode to patients' bodies to record electrical impulses from the heart to perform continuous ECG Monitoring.Performs continual and simultaneous surveillance for the operation of complex cardiac monitoring systems, including ECG rhythm screens, computerized dysrhythmia detection system and rhythm printers.Activates emergency response or rapid response for lethal arrhythmia or cardiac arrest.

Summary:
The Utilization Management RN uses evidence-based medical necessity screening tools to complete initial and continued stay reviews in determining appropriate level of patient care, appropriateness of tests/procedures, and an estimation of the patient’s expected length of stay. The Utilization Management RN follows the Utilization Review (UR) process as defined in the Utilization Review Plan in accordance with the CMS Conditions of Participation for Utilization Review.
Responsibilities:
Addresses system-level issues impeding diagnostic or treatment progress with the healthcare team and reports unresolved opportunities for improvement through the organizational defined escalation process (chain-of-command structure). Proactively identifies and resolves barriers to timely discharge/transition and documents avoidable delays information in accordance with health system protocols.Conducts an in-depth case management assessment of a patient’s needs at the time of admission and throughout the patient’s stay. Obtains and confirms information necessary for the development of a comprehensive discharge/transition plan of care.Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual’s health needs to facilitate a safe and timely discharge to the next appropriate post acute level of care.Collaborates with the Social Worker to proactively identify the need for team and/or patient and family conferences to facilitate discussion of the patient’s condition, discuss prognosis and determine an agreed upon transition of care plan.Consults with Physicians and multidisciplinary teams regularly to evaluate the patient's status and appropriateness of medical care, including admission, length of stay, transfer, and discharge.Performs ongoing chart review to identify actual or potential issues, which may include service delivery, patient outcomes, and satisfaction, compliance, cost, and reimbursement.Actively coordinates progress and the patient’s care by monitoring the length of stay (LOS) of the patient’s hospitalization, leads and facilitates rounds, and proactively works to meet expected length of stay and clinical targets/indicators.Monitors patient and family satisfaction. Summary:
The role of the Entry Point Case Manager is to provide utilization management and safe discharge planning for patients presenting to the Emergency Department (ED) or other entry points in need of care. The appropriate management and deployment of the Common Pool Medical Equipment (CPME) significantly contributes to the overall quality of healthcare delivered at the Medical Center.
The primary function of the Clinical Equipment Coordinator (CEC) is the management of the CPME and its related supplies and accessories to ensure that sufficient CPME devices and their corresponding supplies are available to clinical staff for the administration proper patient care.
The CEC contributes to the success of the business of the Clinical Engineering Department by providing excellent communication and interpersonal skills, supported with a positive attitude.
The CEC will be logistically supported by the following departments: Nursing, Materials Management, Transport, and Biomedical Engineering.

KEY RESPONSIBILITIES:

Monitor and maintain par levels of both CPME and their corresponding consumables/ accessories for efficient allocation and deployment by performing daily rounds in the various nursing floors, designated clean and dirty equipment rooms, and the patient rooms that have been designated as home for certain CPME.
Track consumableccessory inventories and submit purchase lists to Materials Management to maintain necessary operational inventory.
Respond to all service requests for CPME. The CEC will provide first look response to all service requests and will determine if problem is equipment, operator, or a procedural error. The CEC will send equipment to Biomedical Engineering for testing, repair, and upgrades.
Ensure Preventative Maintenance inspections occur on time for CMPE by partnering with Biomedical Engineering.

The appropriate management and deployment of the Common Pool Medical Equipment (CPME) significantly contributes to the overall quality of healthcare delivered at the Medical Center.
The primary function of the Clinical Equipment Coordinator (CEC) is the management of the CPME and its related supplies and accessories to ensure that sufficient CPME devices and their corresponding supplies are available to clinical staff for the administration proper patient care.

Maintain an accurate and dynamic inventory of all CPME throughout the medical center.
Clean and tag equipment to established standard.
Monitor and maintain par levels of both CPME and their corresponding consumables/ accessories for efficient allocation and deployment by performing daily rounds in the various nursing floors, designated clean and dirty equipment rooms, and the patient rooms that have been designated as home for certain CPME.
Track consumableccessory inventories and submit purchase lists to Materials Management to maintain necessary operational inventory.
Respond to all service requests for CPME. The CEC will provide first look response to all service requests and will determine if problem is equipment, operator, or a procedural error. The CEC will send equipment to Biomedical Engineering for testing, repair, and upgrades.
Ensure Preventative Maintenance inspections occur on time for CMPE by partnering with Biomedical Engineering.
