_*Summary of Responsibilities*_

The Physical Therapist Assistant will use their professional skills to assist and monitor the progress of pediatric patients based on their individualized plan of care. The Occupational Therapist Assistant (COTA) will treat patients in their place of residence who are recovering from injuries, illnesses, or disabilities and based on the plan of care to reach the therapeutic goals of everyday activities as determined by the physician orders and other healthcare team members. Advanced Home Health is looking for a Full-Time or per diem Occupational Therapist / OT to join our Home Health Team.

A Home Health Occupational Therapist works closely with the patient care team: RN, LVN, PT, PTA, OT, COTA & HHA.

Evaluates patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self care ability, work capacity, etc.).

Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs.

Occupational Therapist participates in the development of the total plan of care.

Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other organization professionals.

May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.
