$20.80 - $31.20Major Responsibilities:
- Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
- Checks in and registers patients; obtains and verifies complete demographic, guarantor, and insurance information; discusses and collects co-pays and other out-of-pocket patient responsibilities. Obtaining accurate information at the point of registration helps ensure timely payment to the organization and prevents billing issues and patient complaints. Maintains complete confidentiality regarding patient personal/financial information and medical records in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
- Knows insurance basics and recognizes commercial and government plans. Understands which plans Advocate Health contracts with and when a statement of financial responsibility is needed. Understands and discusses financial information and obligations with patients. Knows how and when to refer patients to Financial Advocates.
- Has knowledge of which rules, forms and questions must be enforced to make sure Advocate Health remains compliant with government agencies and regulations. Examples are HIPAA, Emergency Medical Treatment and Active Labor Act (EMTALA), Consent for Treatment, Patient Rights and Responsibilities, Important Message from Medicare (IMM), Medicare Outpatient Observation Notice (MOON), Notice of Privacy Practices, Medicare Secondary Payer Questionnaire (MSPQ), Advanced Beneficiary Notice (ABN). Obtains patient or guarantor signatures as required.
- May schedule patient appointments: may also coordinate cancellations, reschedules, wait list requests, and recall requests. Provides accurate, detailed information regarding test preparations, patient arrival time, medication/food/beverage consumption guidelines, check-in procedures, directions to facility, etc.
- Creates a welcoming and professional environment for our patients and visitors by demonstrating extraordinary customer service. Greets patients and visitors and responds to routine requests for information. Answers telephone, screens calls, and takes messages. Offers various assistance to patients to include arranging transportation needs, providing directions, locating a wheelchair, coordinating interpreter services, etc.

- Educate patients on the insurance coverage aspect of their care including managing the discussion for services that will not or may not be paid by their health plan.

- Collect and manage payments including cash payments and follow security related to cash handling.
- Strong understanding and comfort level with computer systems. Demonstrated technical proficiency including experience with insurance verification/eligibility tools, EPIC electronic medical record, patient liability estimation tools, electronic email, Microsoft Office, internet browser and phone technology.
- Understanding of basic medical and insurance terms and abbreviations typically used in the patient scheduling and registration process.
- HIPAA-compliant and knowledgeable of applicable state and federal rules/regulations. Ability to handle sensitive and confidential information according to internal policies.
- General understanding of health insurance: Medicare, Medicaid, managed care, and commercial payers. Knowledge and ability to articulate explanations of Medicare/HIPAA/EMTALA rules and regulations and comply with updates on insurance pre-certification requirements.
- Excellent organizational skills.
- Demonstrated ability to effectively act as a resource to other staff.
Physical Requirements and Working Conditions:
- This position may require travel, therefore, will be exposed to weather and road conditions.
- Responds quickly to patient medical status and emergency situations occurring in clerical areas.
- CPR certification may be required based on service location.
- Notary training/licensure may be required as appropriate for service line and/or facility.
- Operates all equipment necessary to perform the job.
- Exposed to a normal office environment with significant patient and public contact. May be exposed to ill or contagious patients.
- Must be able to transition from sitting to standing frequently. Must be able to stand and sit for extended periods of time and be physically mobile throughout the workday.
- Frequently lifts to 10 lbs. and occasionally lifts 20 lbs. or more. Must be able to push/pull up to 50 lbs. with assistance. Occurs when moving equipment, supplies and/or when transporting patients through doorways, ramps and elevators.
- Escorts patients to nursing units or outpatient departments as necessary.
- Sensory requirements include vision, hearing and touch. Must also be able to speak clearly.
- Must be able to use hands with fine motor skills for keyboard data entry.
- May be asked to work a flexible schedule at times to meet the needs of the department.

Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. May conduct hospice initial and comprehensive assessments and establish and update the plan of care. Ensures continued skilled nursing/home bound criteria are met. Initiates appropriate preventive and rehabilitative nursing procedures.
Utilizes proper body mechanics, assistive devices and safety techniques when performing various patient positioning/repositioning and transporting duties, which require lifting, and pushing/pulling. Orients and mentors new staff as requested.

This includes the operating table, autotransfusion system, electrocardiograph (ECG) and temperature cables, hyper/hypothermia blanket, medication and pressure manifolds, rapid infusion warmer, recorders, defibrillators, the patient monitoring system, and transesophageal echocardiography imaging unit (TEE) machines.
- During surgical procedures, assists in the insertion of arterial pressure lines, central venous and Swan-Ganz catheter insertion, draws arterial and/or venous blood samples for diagnostic testing, checks ventilators for proper operating efficiency, operates monitor and recording systems, and identifies the various types of intubation blades and fiberoptic bronchoscopes as well as any other special equipment or supplies required for difficult intubation.
- Calibrates all pressures and recording systems within the patient monitoring system to ensure and maintain precise readings.
- Assists in transfers of patients to and from the operating room, collects all data sheets and transports them with the patient, and establishes ECG and pressure monitoring at the destination.
- Cleans all non-disposable equipment after each procedure, performs daily maintenance checks and stocks equipment.
- Moves anesthesia equipment to proper surgical suite; checks equipment to verify proper connections, gases turned off, etc.
- Maintains adequate supply of proper equipment such as: needles, syringes, air-ways, endotracheal tube (ET) tubes, scopes and blade, stethoscope, blood pressure cuff and other necessary items in anesthesia carts.
- Troubleshoots equipment malfunctions or failures and takes all necessary steps to promptly return the equipment to a safe and reliable operating condition, using backup equipment when necessary.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

Aurora - Burlington Medical Center located in Burlington WI is looking to fill a class with students that would transition into employment as a CNA at our Burlington Medical Center.
Training is held onsite at Aurora-Summit in Oconomowoc (36500 Aurora Drive, Summit, WI 53066) with the exception of a practice day and testing date that is held near our St. Luke's campus in Milwaukee.
Aurora Health’s CNA training program is a Wisconsin approved program for education and training to be prepared for Wisconsin Certified Nursing Assistant Certification Exam. In order to meet the requirements for the state certification exam all 88 hours must be completed. PERFECT ATTENDANCE IS REQUIRED (including being on time for every class.)
Compensation: You will be paid $19.80/hr during class and clinicals. Once you pass your CNA exam and transition into the CNA role, you will receive an increase in pay to $20.80/hr.
Benefits: You will become benefit eligible once you are placed into a CNA position.
Once the CNA training class is completed, you will be assigned position as a CNA at Aurora-Burlington. You will want to ensure that the medical center in Burlington is a long term fit as we will not make exceptions to our transfer guidelines to other sites.
Major Responsibilities
Adheres to syllabus rules, attends all 88 hours of training, completes all classroom assignments and passes final exam with at least a 75%.
Upholds Aurora's vision, mission, and policies and procedures.
Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
Measures and documents TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR) with instructor support.
Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately with instructors support.
Collects urine, stool, and sputum specimens as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection and assistance in eating.
Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to observe data reflective of the patient's status and report the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. - Positions patients and selects exposure factors for various radiologic examinations, and provides patient with radiation protection and safety in accordance with prescribed methods.
- Identifies patient and labels images pertaining to the patient, compares requisitions to physician's orders to ensure exam accuracy, and processes images ensuring quality of radiographs and calls back patients as necessary.
- Assists, communicates and consults with the Radiologists.
- Prepares and maintains records in accordance with various regulatory agencies and enters patient and study information into the radiology information system, and documents billing codes of procedures performed to ensure accurate patient charges.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).

- Administers medications, under physician/provider orders, including: immunizations, antibiotics, pain medications, vitamins, topical agents, and insulin. With limitations, initiates, maintains and discontinues IV therapy; administering selected fluids with pre-mixed medications.
- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.

- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
