- Maintain a safe environment and remain with patient always to provide the appropriate level of observation to patient as delegated by the Registered Nurse.
- Appropriately document information that is required during the patient observation experience.
- Perform handoff communication with the Registered Nurse regarding pertinent patient information.
- Assist nursing team in maintaining a ligature-resistant or ligature free environment as appropriate to the clinical setting.
- Notify and update the licensed nurse of immediate patient safety, patient care, and staff needs.
- Fosters positive and professional communication between patients and staff.
- Adhere to all policies and procedures.
Physical Requirements
Work requires walking, standing, sitting, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak and understand English in good understandable terms. Must have manual and finger dexterity. Intact sense of sight, hearing, smell, touch.
Education, Experience and Certifications.
High school diploma or GED required. Previous hospital experience preferred. Basic Life Support Certification required. CPI certification required prior to conducting patient observation.

$44.15 - $66.25- Provides video consultation/triage to patient, family, and significant others according to the Home Health Virtual Program plan of care.
- Provide ongoing care via video according to the established plan of care. Determines priorities of patient care based on essential patient needs and available resources.
- Recognizes changes in the patient's status, need for care and intervenes appropriately.
- Coordinates patient interventions such as physician contacts, consultation with patient's home health care team, , facilitation of appropriate visit strings, changes in physician orders, and care plan goals, in cooperation with the patient’s physician, as appropriate.
- Assesses patient, family and/or caregiver’s training needs and provides teaching/counseling video consultation/triage.
- Collects and maintains clinical and patient records according to agency, state, and federal guidelines. Assumes primary responsibility for video documentation, data review, quality, and appropriate follow-up of patient information.
- Assists in completion of reports and statistics related to key clinical performance measures.
- Ensures quality of care through development, implementation, and evaluation of professional standards.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration and/or Certification Required:
- Physical Therapist license issued by the state in which the teammate practices. If the teammate supports multiple states, licensure in at least one of those states is required prior to hire.
Education Required:
- Bachelor of Science, Masters, or Doctorate Degree in Physical Therapy
Experience Required:
- Typically requires 5 years of experience in combination of rehab, home care and triage settings.
Knowledge, Skills & Abilities Required:
- Must possess strong communication skills, be detail oriented and have the ability to coordinate and manage complex care for patients over video.
- Must be motivated and self-directed and possess above average interpersonal skills.
- Demonstrated ability to work well with management, physicians, and other professionals in a direct and positive manner.
- Demonstrated ability to work in a fast-paced environment and under time constraints.
Advanced computer skills using Microsoft Office applications and an overall proficiency and comfort level with computer systems.

Physical Requirements
Duties require working, transporting, and driving in all weather conditions to include exposure to extreme hot, cold, and wet weather. Must not have any physical, emotional, or mental impairment to prevent them from riding, driving, or administering patient care in a transport vehicle. This work area may have cramped space and be poorly lit. Noise levels are moderate as associated with working in and around emergency.
Hearing protection is available and required for use around “hot” aircraft. $54.90 - $82.35This teammate will be responsible for assisting the multidisciplinary team with medication therapy selection and management in areas including but not limited to trauma, medical resuscitations, and medical emergencies in STEMI, stroke, PE, and sepsis. The candidate will work with the clinical team leader with protocol development and education to the medical staff as necessary. The candidate will be expected to perform limited preparation and verification of medications in high acuity situations.
Major Responsibilities:
Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format.
Reviews and accurately interprets medication orders and prescriptions
Participates in the management of medical emergencies when applicable
Actively supports department medication safety and quality improvement activities, including the reporting of events through appropriate channels
Performs quality assurance and performance improvement of the medication use system as appropriate
Collaborates with the healthcare team to manage formulary adherence and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
Practices in compliance with organization’s policies and procedures, as well as local, state, and federal regulations
Prepares, dispenses, and oversees the proper storage and handling of medications in accordance with best practice, policies and state and federal regulations
Serves as preceptor for pharmacy students, residents, and healthcare learners as assigned
Mentors, orients, trains, and assists with the performance evaluation of pharmacy teammates, residents, and students as assigned
Directs and checks the accuracy of the work of pharmacy support team members in accordance with the organization’s policies and procedures to ensure safe and efficient workflows, service standards, and regulatory compliance
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. The APP assesses and coordinates the educational and psychosocial needs of the patient and caretakers. The APP develops and reviews the plan of care with the other members of the clinical team, participates in directing that care, and coordinates discharge planning/and or follow-up (as applicable). The APP participates in the education of residents and medical/nursing students and performs diagnostic procedures and therapeutic procedures.
Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Such service may include but are not limited to.
MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:

JOB SUMMARY:
Independently plans and provides professional nursing care for patients in the ambulatory clinic settings in accordance with the medical and nursing plans of care and established policies and procedures. Provides nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing. EDUCATION/EXPERIENCE:
Graduation from accredited School of Nursing. BSN preferred. Two years of acute care experience required. Prefer acute care experience within the last 10 years.
LICENSURE, CERTIFICATION, and/or REGISTRATION:
Current license as a Registered Nurse (RN) in the State of North Carolina and Basic Cardiac Life Support (BCLS) required. Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) or Pediatric Emergency Assessment, Recognition and Stabilization (PEARS) may be required within one year in select areas.
