We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team. The ideal candidate will provide high-quality patient care in our relaxed, homelike long-term care facility. He or She will work closely with the D.O.N. and support staff to ensure the well-being of our residents and administer medications.
*Duties*
* Administer medications and treatments as prescribed by physicians.
* Perform patient assessments and monitor vital signs.
* Assist with daily living activities and provide personal care to patients as needed.
* Document patient information accurately: charting only for exception.
* Provide education and support to patients and their families regarding health management.
* Maintain a safe environment within our residents' home.

- Under the direction of the operations management team, provides appointment scheduling ranging in complexity from one physician appointment to multiple care provider appointments in accordance with department scheduling guidelines for new and/or existing patients.
- Performs front desk check-in and check-out functions
- Obtains detailed clinical information face-to-face, by telephone or electronically.
- May provide basic information regarding the need for insurance referral(s).
- Performs independent patient and outside provider call triaging and assists in directing patient flow.
- Serves as a liaison between patient/family/provider.
- Confirm patient’s demographics (address/phone numbers etc) and insurance information.
- Review patient “alerts” in Epic and reconciles ie: patient contact information, Medicare Survey etc.
- Responsible for complex scheduling activities associated with patients a
- rriving from main campus, other centers or other outside entities and coordinating appointments with other specialists, dieticians, social work, 2nd opinions, etc.
- Ensures the completion of prior authorizations for radiology testing and authorizations for outside ancillary services if requested same day or within 24 hours.

- May navigate new patients and educate them about all available services.

- Reviews prepared lists for patients needing a Health Care Proxy or Illness Impact Questionnaire to be completed. Assists those patients in completion of forms. Answers non-clinical questions about the Health Proxy or refer any detailed.
- Coordinates care with main campus and facilitate appointments. Tracks referrals and generate reports to evaluate with entities are sending patients.
- Serves as the first point of contact and liaison for patient/family/provider
- Provides administrative support and coordination for all aspects of patient care for patients, including supporting incoming telephone calls.
- Triages issues and answer general questions, with the goal of resolving requests in real time. If unable to answer all patient questions, triage to or take detailed message for care team.
- Provides general institute, disease, or program-specific information to patients/callers within the scope of knowledge and authority.
- Collaborates with satellite team to ensure seamless coverage and task management in times of both full and partial staffing levels.
- Ensures quality clinical care and adherence to standard operating procedures and compliance requirements.

- Delivers expert, evidence-based nursing care to patients and families: Implements nursing process: Assessment, Nursing Diagnosis, Planning, Implementation, Evaluation. Performs the following technical skills; Intravenous therapy including peripheral and central venous vascular access devices, including obtaining IV access, Safe handling practices for hazardous, cytotoxic and biological agents, Chemotherapy administration skills, including vesicant and non-vesicant medications. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role. Supports the DFCI Nursing Department Professional Practice Model and Quality Plan. Accurately records charges for all services rendered on the day service. Collaborates with Clinic Assistant to assure accurate inventory of all supplies. Serves as a resource to medical personnel in other health facilities, providing telephone consultation to engender optimal Oncology practices and knowledge among health care professionals caring for cancer patients.
- Establishes rapport with patients and families from a variety of cultural, ethnic and religious backgrounds: Interacts with patients and families in a caring and compassionate manner respecting the need for privacy and confidentiality. Develops approaches to problem solving which meet the individual needs of the patients and families. Demonstrates the knowledge and skills necessary to provide age-specific care and support to the populations served. Promotes a culture of inclusion. Fosters an environment that is sensitive to the needs of diverse populations, including but not limited to culture, ethnicity, gender, and age.

MINIMUM JOB QUALIFICATIONS:
The position requires licensure as a Registered Nurse in the Commonwealth of Massachusetts or New Hampshire, as appropriate. A bachelor’s degree in nursing is strongly preferred, and candidates with an associate degree must complete a BSN within 4 years. Completion of the Oncology Nurse Residency Program (ONRP) or 1 year of recent experience as a Registered Nurse. Ambulatory and/or oncology experience preferred. Professional nursing certification (e.g., OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC) is preferred, and AHA BLS certification is required (AHA ACLS is acceptable as a minimum requirement).
SUPERVISORY RESPONSIBILITIES:
No direct supervision of others but does supervise assistive personnel as appropriate in the provision of patient care.

21. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role.Supports the DFCI Nursing Department Professional Practice Model and Quality Plan.
22. Accurately records charges for all services rendered on the day service.
23. Collaborates with Clinic Assistant to assure accurate inventory of all supplies.
24. Serves as a resource to medical personnel in other health facilities, providing telephone consultation to engender optimal Oncology practices and knowledge among health care professionals caring for cancer patients.

- Delivers expert, evidence-based nursing care to patients and families: Implements nursing process: Assessment, Nursing Diagnosis, Planning, Implementation, Evaluation. Assesses patient and family needs. Promotes health care outcomes in concert with currently accepted clinical practice guidelines. Analyzes assessment data to develop and support clinically accurate diagnoses. Collaborates proactively with all interdisciplinary team members with a patient centered focus to facilitate and maximize patient healthcare outcomes. Plans care by establishing goals with measurable outcomes. Implements the plan of care. Evaluates the patient’s response to interventions. Documents care according to Institute and regulatory agencies’ requirements. Administers medications and other treatments, which may include chemotherapy, and performs procedures safely and efficiently in accordance with Institute policy. May administer blood transfusions. Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care. Educates patient and family regarding treatment modalities of disease, which may include chemotherapy and radiation therapy and symptom management. Reviews patient’s clinical status and laboratory values pre-treatment and consults with provider when appropriate. Evaluates and determines need for patient referrals to other members of the interdisciplinary needs as appropriate. Identifies appropriate community resources to promote continuity of care and makes appropriate referrals. Manages patient care across the continuum, making appropriate referrals to home care and hospice, as necessary. Prepare and administers chemotherapy/biotherapy and supportive therapy in accordance with ASCO/ONS Guidelines. Recognizes and responds to emergency situations appropriately. Maintains BLS certification. Provides telephone triage according to policies and documents telephone interactions. Performs the following technical skills; Intravenous therapy including peripheral and central venous vascular access devices, including obtaining IV access, Safe handling practices for hazardous, cytotoxic and biological agents, Chemotherapy administration skills, including vesicant and non-vesicant medications. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role. Supports the DFCI Nursing Department Professional Practice Model and Quality Plan. Accurately records charges for all services rendered on the day service. Collaborates with Clinic Assistant to assure accurate inventory of all supplies. Serves as a resource to medical personnel in other health facilities, providing telephone consultation to engender optimal Oncology practices and knowledge among health care professionals caring for cancer patients.
- Establishes rapport with patients and families from a variety of cultural, ethnic and religious backgrounds: Interacts with patients and families in a caring and compassionate manner respecting the need for privacy and confidentiality. Develops approaches to problem solving which meet the individual needs of the patients and families. Demonstrates the knowledge and skills necessary to provide age-specific care and support to the populations served. Promotes a culture of inclusion. Fosters an environment that is sensitive to the needs of diverse populations, including but not limited to culture, ethnicity, gender, and age.
- Manages the environment of patient care: Monitors the environment of care with attention to patient safety and assures compliance with regulatory agency standards. Complies with applicable policies, procedures, work rules and standards of conduct. Adheres to the Code of Ethics for Nurses with Interpretive Statements (ANA, 2015) in all aspects of professional practice. Bases care on the Standards of Oncology Nursing Practice and Oncology Nursing Society Chemotherapy Guidelines.
- Works collaboratively and functions as an effective member of the health care team: Demonstrates the ability to work collaboratively with peers and co-workers. Effectively addresses and manages conflicts. Demonstrates excellent communication skills. Demonstrates effectiveness as a contributing member to the multidisciplinary health care team. Delegates as appropriate and supervises assistive personnel in their provision of patient care. May function in the charge nurse role when necessary, collaborating with staff and coordinating flow of patients into the infusion areas. Participates in committees, councils, and initiatives. Demonstrates flexibility in setting and changing priorities. Accepts additional responsibilities as needed. Participates in QA/QI projects for the unit as requested.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

- IV skills strongly preferred
- Work tasks require walking, standing, climbing, sitting, bending, reaching and occasional lifting. Some tasks are performed while sitting

- Communications with physicians or staff during office hours requires organization of information, discretion and balancing of multiple communications

MINIMUM JOB QUALIFICATIONS:
The position requires licensure as a Registered Nurse in the Commonwealth of Massachusetts or New Hampshire, as appropriate. A bachelor’s degree in nursing is strongly preferred, and candidates with an associate degree must complete a BSN within 4 years. Completion of the Oncology Nurse Residency Program (ONRP) or 1 year of recent experience as a Registered Nurse. Ambulatory and/or oncology experience preferred. Professional nursing certification (e.g., OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC) is preferred, and AHA BLS certification is required (AHA ACLS is acceptable as a minimum requirement).
SUPERVISORY RESPONSIBILITIES:
No direct supervision of others but does supervise assistive personnel as appropriate in the provision of patient care.
