Education
Required – High school diploma or equivalent.
Work Experience
Required – None.
Preferred – Work experience in a materials management or healthcare environment.
Certifications
Required – Valid driver’s license if operating a company vehicle.
Forklift certification required if working at Warehouse District Row location within 30 days of hire.
Preferred – Supply chain or inventory management certification(s).
Knowledge Skills and Abilities (KSAs)
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and ability to present information clearly and professionally.
Strong interpersonal skills.
Ability to travel throughout and between facilities.
Ability to work flexible schedule (i.e. 24/7, weekend, holiday, on call availability).
Job Duties
Maintains inventory and cleanliness in storeroom and par locations.
Ensures supplies are accurately charged when removing from inventory.
Efficiently organizes delivery carts according to delivery locations.
Delivers according to designation on receiving report.
Maintains an accurate delivery log for non P.O. deliveries.
Gains a legible signature for all deliveries.
Inspects and receives deliveries for hospital, clinic and administrative areas.
Performs all tasks related to inventory, including cycle counts as well as complete end of year inventories.
Identifies and addresses internal and external customer needs.
Picks up and delivers supplies, equipment, mail, pharmaceuticals, and other related materials to/from campus to designated offsite locations and maintains accurate delivery logs as required.
Performs other related duties as required.

Physical and Environmental Demands

Very Heavy Work - Exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
Duties performed routinely require exposure to blood, body fluid and tissue.
The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases. There may be an occupational risk for exposure to communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.

Education

Supports the plan of care as delegated by staff nurse within scope of skills verification.
Maintains a clean and safe environment in the patient care area.
Provides care based on physical, psychosocial, educational, safety and related criteria, appropriate to the ages of patients served in assigned areas.
Communicates patient care needs effectively with patients, families, and other staff members, including multi-disciplinary teams.
Completes documentation of patient related tasks.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Education
Required - Graduate of an accredited school of nursing.
Work Experience
Required – 3 years of nursing experience
Preferred – Experience in home health or clinic setting
Certifications
Required – Current registered nurse (RN) license in state of practice.
Current Basic Life Support (BLS) certification from the American Heart Association.
Knowledge Skills and Abilities (KSAs)
- Good organizational and time management skills and ability to be self-directed and demonstrate good judgement.
- Good interpersonal skills.
- Ability to work with and maintain confidential information.
- Ability to work in a fast-paced environment with minimal supervision.
Job Duties
- Provides direct patient care, evaluates outcomes, assesses risk factors and patient needs, and consults with other health team members as required under the supervision of a NP and/or physician.
- Determines what preventive services the patient qualifies for utilizing the appropriate Medicare/Medicare Advantage portal tools. Educates the patient and family regarding these services and any available resources.
- Provides continuity by ensuring accurate appointment type set-up, prepping the paperwork for visit, and performing all aspects of the wellness visit per policy.
- Ensures the provider has information documented for correct and complete billing of services rendered after face-to-face portion of the encounter. May also assist provider with procedures as needed, both technical and manual, under the supervision of a NP and/or physician.
- Focuses on identifying and addressing chronic disease diagnoses, particularly Hierarchical Condition Categories (HCC), to support effective risk management and ensure comprehensive patient care.
- Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
- Performs other related duties as assigned.

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. Duties performed routinely require exposure to blood, body fluid and tissue. The incumbent works in a patient care area; works in an area where patients enter; works directly with patients; and/or works with specimens that could contain communicable diseases. The incumbent has an occupational risk for exposure to all communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.
