This position provides support to the Care Management Team to ensure that the day to day operational functions of the department are met in a timely manner. This position includes collaboration with third party payers as well as post-acute providers. This position will also be responsible for timely and appropriate documentation within the electronic Care Management System.MINIMUM QUALIFICATIONS:

1. Two (2) years administrative experience.

1. Associates or advanced degree.

1. Two (2) years’ experience in a health care setting.
2. Hospital, home health, or payer relations experience.
3. Clerical experience in utilization management, discharge planning or case management.
4. One (1) year of ICD9/CPT Coding or Medical Terminology experience.

1. Greets visitors and health care personnel promptly and professionally; screens visitors and telephone calls to determine urgency of contact and to facilitate appropriate routing or referral; representing Care Management in a positive manner.
2.Effectively communicates clinical information with payer to obtain authorization for post-acute services/medications and documents interactions in patient’s electronic medical record (EMR) along with timely communication with Care Management staff.
3. Participates in discharge planning activities (e.g. contact of post-acute care facilities, other community resources and transportation) as needed to ensure a timely patient discharge and appropriate linkage with post-acute providers
4. Provides prompt feedback regarding payer determinations to Supervisor/Care Manager, enabling them to evaluate/redirect the current patient plan of care in order to streamline the delivery of service.
5. Function as placement specialist, as required ,to include research for preferred providers for HHC/DME/Acute Rehab/SNF/LTACH placement
6. Proactively communicates any change in payer information to Supervisor/Care Manager and documents changes appropriately
7. Verify with patient financial counseling that all payment sources have been explored and updated in the patient’s electronic medical record (EMR).
8. Participates in reimbursement, certification and authorization related activities (e.g. faxing or copying required information) as required.
9. Contacts and coordinates with referral agencies to arrange provision of ordered equipment and associated services when appropriate, as directed by Care Managers.
10. Collects copies and transmits pertinent clinical and patient demographic information required to complete arrangements for post-discharge care and/or placement, as directed by the Care Managers.
11. Coordinates and arranges transportation and community services, as directed by care managers.
12. Provides secretarial and clerical support, including faxing, copying charts, filing, typing and scanning.
13. Ensures payer and customer satisfaction through effective communication and positive customer service skills at all times.
14. Maintains confidentiality of communications.
15. Using independent judgment prioritizes appropriately to ensure efficient utilization of time.
16. May serve as back up to the Administrative Assistant for Care Management in their absence.

Job Summary
Position responsible for recommending and administering basic and advanced therapeutic and diagnostic procedures, including arterial blood gases, neonatal procedures, trauma care and ventilator transports, to patients with respiratory disorders, including patients in adult and neonatal critical care units. Job Description
Minimum Qualifications
Education: Bachelor’s degree preferred.
Licenses/Certifications:
- Certified Respiratory Therapist (CRT). Registered Respiratory Therapist (RRT) preferred.
- Current Respiratory Care Practitioner License or Temporary Respiratory Care Practitioner License.
- Basic Life Support Certification
- Two (2) of the following are preferred: ACLS, PALS, NRP, CPFT, RPFT, RPSGT, Asthma Certified Educator, NRP Instructor, ACLS Instructor, PALS Instructor, or BLS Instructor.
Experience / Knowledge / Skills:
- RCP I <1 year
- RCP II = 1-3 years
- RCP III = 3-10 years
- RCP IV = 10+ years
- Two (2) years of critical care experience, preferred.
- Effective oral and written communication skills.
- The position requires occasional lifting and carrying of up to 50 pounds; prolonged standing; frequent walking, stooping and squatting.
- The visual ability to monitor instruments, computers and equipment, recognize alphanumeric characters, distinguish colors and observe physical characteristics are essential to this position.
Principal Accountabilities
- Performs duties in a manner to promote quality patient care and customer service, while promoting safety, cost efficiency and a commitment to quality care.
- Performs respiratory care procedures: administer medical gases, medication, and aerosols for therapeutic and diagnostic purposes and provide patient education.
- Follows written orders from physicians in administering drugs and medication to the cardiopulmonary system of patients, evaluate treatment provided and take appropriate action.
- Performs patient assessment and basic interpretation of arterial blood gases, pulmonary function testing and chest x-rays as indicated in Skills Check Off list.
- Provides expert respiratory advice and recommendations to physicians and other staff for population served.
- Communicates effectively with professional colleagues. Manages time effectively. Documents appropriately in the medical records and completes other documentation duties accurately and efficiently according to policies and procedures.
- Remains current in respiratory care through continuing education, participation in services, and review of journals. Assists in clinic education efforts for patients, staff, hospital, system and/or the community.
- Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
- Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

As a phlebotomist, you'll support Geisinger’s mission of providing exceptional patient care through innovative, efficient, and high-quality laboratory services. At Geisinger, our phlebotomists are the friendly faces that collect samples to be analyzed by our state-of-the-art labs. - Attains 10 points in any of the domains
Level 5
- Meets requirements for previous levels
- Available to those with a BS or higher
- Available to Registered Respiratory Therapists
- Seven years of experience, plus one specialty credential
- Attains 12 points in any of the domains
