Position Purpose: Ensure safe and prompt transportation for PACE participants, including meal delivery and essential supplies. Maintain vehicle cleanliness and safety while promptly reporting any issues to the department leader.
What You'll Do:
- Safely transport PACE participants, prioritizing their comfort and satisfaction.
- Adhere to schedules, adjusting for unforeseen changes and participant needs.
- Operate vehicles in compliance with regulations and safety standards.
- Provide physical assistance to participants as needed during boarding and disembarking.
- Handle biohazard materials with proper precautions.
- Maintain vehicle cleanliness and equipment functionality.
Minimum Qualifications:
- High school diploma or equivalent; Associate's degree preferred.
- Valid driver's license with a clean record; CDL with passenger endorsement for larger vehicles.
- Minimum one year of experience with elderly or frail populations preferred.

Interprets physician referrals and evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living.
Establishes a plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Provides and/or fabricates assistive devices and/or splints to enhance patient's functional independence. Monitors and evaluates outcomes and appropriately adjusts individualized treatment plan based on patient's response and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Aurora Summit Surgery is a high-volume, fast-paced OR supporting approximately 25 cases per day across 12 operating rooms, including two dedicated trauma rooms and two dedicated robotic rooms. The department cares for both inpatient and outpatient elective and trauma patients across a wide range of specialties including trauma, orthopedics, ENT, OBGYN, general surgery, neuro, plastics, ophthalmology, pain, and cysto, with orthopedics making up nearly 50% of overall volume. Unique features include DIEP flap and bariatric specialties, a portable cysto room, newly renovated OR suites upgraded in 2026, and two additional rooms added to support growth. Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.

Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.

Provide care for infants at 32 weeks of gestation or later weighing 1500g or more who have physiologic immaturity or who are moderately ill with problems that are expected to resolve rapidly and are not anticipated to need subspecialty services on an urgent basis. Provide mechanical ventilation for brief duration (less than 24 hours) or continuous positive airway pressure or both.

Education Qualifications
- Associates degree in allied health, applied/radiologic science or certificate program is required.
Experience Qualifications
- 1 year of radiology experience is preferred.
Certifications, Licenses and Registrations
- WI: Licensed Radiographer issued by the WI Department of Safety and Professional Services (DSPS) is required.
- IL: Accreditation (or Temporary Accreditation) in Medical Radiation Technology issued by the IL Department of Emergency Management Agency (IEMA) is required.
- Certification/registration or registry-eligible as a Radiologic Technologist in Radiography (RT(R)) by the American Registry of Radiologic Technologists (ARRT) is required. Certification/registration must be obtained within 12 months.
- Computed Tomography (CT) credential from ARRT is preferred.
- Basic Life Support (BLS) is required.

What You Will Do:
- Provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders.
- Complete participant nutritional assessments and reassessments as needed.
- Participate as a member of the interdisciplinary team (IDT), conducting initial medical history, physical exams, and functional nursing assessments of each new participant, as well as semi-annual, annual, and unscheduled assessments.
- Participate in daily program activities and provide comprehensive care plans to meet participant needs and promote optimum health, independence, and self-care.
- Facilitate the integration of new participants into the PACE program healthcare delivery system.
- Demonstrate nursing care to and instruct participants and families in treatments and self-care interventions.
- Communicate, counsel, and guide participants and families regarding service needs and concerns, providing education on health-related topics.
Minimum Qualifications:
- Graduate of an accredited Nursing Degree program with a current, unencumbered, active RN license in the state of Maryland
- BSN preferred. Active CPR Certification required.
- Minimum of two (2) years of demonstrated successful experience in related services with the geriatric population.
- Valid driver's license in good standing and proof of insurance required.
- Ability to perform standard clinical procedures according to established protocols, with initiative, solid problem-solving skills, and thoroughness in documentation.
- Superior written, verbal communication, and presentation skills, able to effectively communicate with diverse perspectives.
- Proficiency with Microsoft Office – Word, Excel, Outlook, and other required software.
- Flexibility in work priorities with frequent physical tasks and lifting up to 20 pounds.

Physical Requirements & Working Conditions
Requires frequent sitting, standing, walking, bending, squatting, reaching, twisting, and lifting throughout the workday.
