*JOB SUMMARY*
The primary purpose of this position is to be responsible for providing skilled speech, language, and swallowing therapy services. The speech-language pathologist / SLP assesses and develops individualized treatment plans and provides speech, language, and swallowing interventions to enhance residents' quality of life and functional independence.
Job responsibilities may include but are not limited to:
Speech assessments and evaluations
Collaborate with other health care professionals in the assessment, planning, and safe implementation of all therapy.
Be a resident advocate where the resident is the center of all we do.
Ensure excellent communication among residents, families, and staff.
Provide clinical documentation
Create positive resident outcomes.
Respect others by treating them the way we want to be treated.
Demonstrate kindness, compassion, honesty, and integrity at all times.
Help create a culture of superior customer service.

*REQUIREMENTS*
Graduate of an accredited speech-language pathology / SLP program

Our practice has served the community since 2000 and offers physical therapy and wellness services under one roof, including orthopedic PT, pelvic health, Pilates, postpartum care, and incontinence therapy. Our pelvic health treatment model is private, confidential, one-on-one, and rooted in empathy, education, and patient trust.

Required Qualifications:
Associate degree from an accredited Occupational Therapy Assistant program.
Current state licensure as a Certified Occupational Therapist Assistant (COTA).
Strong interpersonal and communication skills, with a passion for working in long-term care settings.
Ability to work collaboratively within a multidisciplinary team.
Major Duties and Responsibilities:
Assist the Occupational Therapist in implementing individualized treatment plans to enhance residents’ physical, emotional, and cognitive functioning.
Provide hands-on therapeutic interventions, including exercises, activities of daily living (ADL) training, and adaptive equipment use.
Document residents’ progress and communicate updates to the care team to ensure continuity of care.
Promote a safe and supportive environment while fostering meaningful relationships with residents.

Responsibilities
* Perform patient evaluations to determine cause and severity of chronic pain.
* Order and interpret diagnostic tests MRI/ CT scan, X-Ray, labs to support diagnosis and treatment planning.
* Provide interventional pain management treatments and low-dosage medication management.
* Educate patients on conditions, procedures, and long-term management strategies of all Pain Management procedures .
* Maintain accurate documentation and compliance with federal and state regulations - follow CMS guidelines
* Stay current on research, best practices, and trends in Pain Management.
