As a Home Health Physical Therapist, you’ll deliver one-on-one patient care while playing a critical leadership role in supervising and guiding Physical Therapist Assistants (PTAs). Your expertise ensures every patient receives safe, effective, and goal-driven care—every visit, every time.
This is your opportunity to practice at the top of your license, with the autonomy to make clinical decisions and the responsibility to oversee the full continuum of therapy care.
Your Impact:
Perform comprehensive evaluations and establish clinical diagnoses
Develop and implement individualized treatment plans focused on functional outcomes
Directly supervise PTAs, ensuring treatments align with the plan of care and regulatory standards
Provide ongoing guidance, mentorship, and clinical oversight to PTAs
Conduct reassessments and supervisory visits to ensure patient progress and quality outcomes
Maintain accurate, compliant documentation for both your care and PTA services
Collaborate closely with patients, families, and the interdisciplinary team
Ensure infection control and patient safety standards are consistently followed
Why This Role Stands Out:
Leadership opportunity – take an active role in supervising and developing PTAs
Autonomy + influence – guide patient care from evaluation through discharge
