What You'll Do
Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria's Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
Your Qualifications
Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty. New graduates who are board-eligible and actively pursuing licensing are encouraged to apply. Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Your Goal:
This position is responsible for the provision of high-quality clinical care for the patients of Greater Family Health.
How You’ll Help Patients:
Provide high quality medical care to the patients of Greater Family Health Center in accordance with Specialty Certification and Greater Family Health Policies and Procedures.
Is fully supportive of all quality initiatives.
Is fully receptive to other responsibilities as assigned.
Provides all forms of leadership, supervision, direction, training and role modeling to all supportive staff (eg: NA, MA, LPN, RN, PA, NP).
Works to assure that all responsible areas are fully compliant with all applicable federal and state rules, regulations and laws. Insures that all responsible areas are in compliance with all Greater Family Health objectives, systems, policies, procedures, directives and contractual agreements as indicated by supervisors, contracts, training personnel and Greater Family Health policies and procedures.
Maintains a level of efficiency that indicates satisfactory productivity.
Provides on call coverage and rounding on inpatients (as appropriate).
Provides care in age appropriate fashion according to one’s specialty.
Report to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers. Follow and fully implement Corporate Compliance Program.

What You'll Do

The Role
We are hiring a Psychiatrist (PT) to join our clinic opening in Orange, CA. You would be part of an interdisciplinary team dedicated to delivering personalized, compassionate, high-quality care for individuals with SMI.
What You'll Do
Provide behavioral healthcare management to Amae Health patients; monitor and manage care through an array of treatment modalities as patients progress through phases of recovery
Develop individualized treatment plans during the engagement and stabilization phase, focused on specific psychosocial and medical needs that might limit recovery
Provide continuous, regular consultations as patients navigate recovery, both in person and occasionally virtually
Liaise with patient care teams and collaborate with behavioral health providers to ensure an integrated, cohesive approach to care
Address medical needs to mitigate negative impacts of social determinants of health
What You'll Have
Board-Certified or Board-Eligible Psychiatrist, with a CA license and DEA that are active, unrestricted, and in good standing
In possession of or willing to obtain Clozapine REMS certification and Medicaid enrollment if needed
Prior experience with mental health and/or addiction issues; familiarity with the medical, mental health, and social service systems in the Orange County area

Compensation & Benefits
Amae Health is committed to fair and equitable compensation practices. The base salary for this part-time role will be prorated from a full-time equivalent range of $300,000- $330,000 per year + performance-based bonus opportunity. Primary Purpose
Provide direct primary medical care to all participants with the highest medical standards and ethics in a caring, nurturing atmosphere while keeping with Senior Care Partners P.A.C.E. mission, vision, and values.
Duties and Responsibilities
Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested) and documents in the medical record as required.
Functions as a member of the Interdisciplinary Team (IDT), communicates participant changes to team members, and participates in IDT meetings.
Assists in the development, implementation, and evaluation of the participant plan of care as described in Senior Care Partners P.A.C.E. policy.
Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy.
Provides, promotes, and educates participants and families on preventative health maintenance of their chronic medical conditions and actively participates in family conferences.
Regulates and adjusts participant medications as needed and in a cost-effective manner.
Facilitates consultations with medical specialists as indicated and oversees care rendered.
Collaborates effectively with primary (attending) medical management while an inpatient; Provides updates to the IDT on a regular basis; Judiciously utilizes specialty consultants; Coordinates discharge planning with the IDT and hospital discharge planners.
Manages all of a participants care if admitted to a skilled nursing facility (SNF) to include: Regularly visit the SNF for participant evaluation; Communicates with SNF staff via phone/fax as requested/required.
Participates in committees, work groups as assigned, and Quality Improvement activities as required.
Participates in the coordination of the participant’s 24-hour delivery of care services.
Maintains current accurate documentation of health care services provided, coordinated or contracted.
Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach.
Supervisory Responsibilities
This position has no direct supervisory responsibilities. The Physician works collaboratively with Nurse Practitioners and other healthcare professionals as part of the interdisciplinary care team to support the delivery of patient-centered care.
Qualifications
For an MD/DO or NP Role
Graduate of an accredited School of Medicine, College of Osteopathic Medicine, or accredited Nurse Practitioner program.
Current licensure as a Physician (MD/DO) or Advanced Practice Registered Nurse (APRN) in the applicable state.
Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or applicable Nurse Practitioner specialty certification preferred.
Minimum of one year of experience working with frail elderly, long-term care, hospice, palliative care, or medically complex patient populations.
Demonstrated ability to work collaboratively within an interdisciplinary team to manage complex patient care needs.
Specific Requirements
Must possess a current, unencumbered, active license to practice as a MD or DO in the State of Michigan.
Must possess a current, unencumbered, active DEA license.
Current privileges or the ability to obtain and maintain staff privileges at Senior Care Partners P.A.C.E. contracted provider facilities
Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
Must be able to read, write, speak, and understand the English language and follow written and oral instructions.
Must demonstrate necessary skills and knowledge as outlined in the position specific competency requirements.
Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing medical practices.
Must care, support, and collaborate with everyone involved in participant’s family using the patient-family centered care approach.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to communicate tactfully with personnel, participants, family members, visitors, government agencies/personnel, and the public.
Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.
Must possess the ability to plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to manage difficult participants.
Must be knowledgeable of computers, data entry, output, system applications, etc.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Work Environment
Has a visible presence while working in the center and is subject to frequent interruptions.
Has a working presence in the community and is involved with participants, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
Works beyond normal working hours and on weekends and holidays when necessary.
Subject to callback during emergency conditions (e.g., severe weather, evacuation, disaster, etc.).
Is subject to hostile and emotionally upset participants, family members, personnel, and visitors.
Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or the Hepatitis B virus.
Communicates with staff, participants, caregivers, the board of Senior Care Partners P.A.C.E. and other organizations, as necessary.

Must be able to move intermittently throughout the workday.
Must be able to cope with the mental and emotional stress of the position.
Must possess sight/hearing senses or use prosthetics that will enable these senses to function so that the requirements of this position can be fully met.
Must function independently, have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of Senior Care Partners P.A.C.E., which includes a medical physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the Senior Care Partners P.A.C.E. Program and/or center.
Must be able to lift twenty pounds.
May be necessary to assist in the evacuation of participants during emergency and/or disaster situations.
Minimum Working Requirements
Must have reliable transportation, current valid Michigan driver’s license and car insurance.
Must be 18 years of age or older.

The Community Spay & Neuter Clinic at Dakin Humane Society is the largest of its kind in New England, providing high-quality, accessible spay and neuter surgery for dogs and cats throughout Springfield and the surrounding communities.
Since opening in 2009, we've performed more than 144,000 spay and neuter surgeries, helping reduce pet overpopulation, prevent unwanted litters, and keep more animals where they belong—with the people who love them. As more pets and families turn to Dakin for affordable care, we're looking to grow our surgical team with a dedicated Spay & Neuter Veterinarian who shares our commitment to exceptional medicine and community impact.
In this role, you'll spend your day in surgery, performing up to 30–40 high-quality spay and neuter procedures for community-owned cats and dogs, feral cats, and shelter animals as needed. You’ll work with an outstanding support staff and help make veterinary care more accessible to the families who need it most.
