We have been providing primary healthcare to residents since 2008. Today, you will join a healthcare organization and a staff of 11 providers at four locations, delivering primary medical, dental and behavioral health care to more than 5,500 patients. You will be welcomed into our caring environment to serve the community in which we live.
*We have high expectations for you as a Medical Provider. We need you to:*
* Provide clinical and educational services to clients of the Health Center. Act as a resource to professional and support staff.
* Provides primary care services to patients of all ages. Elicits and screens medical histories, performs physical examinations. Requests and interprets laboratory and diagnostic examinations. Assesses, identifies and evaluates health programs and develops and documents a care plan.
* Develops and/or utilizes written protocols for the performance of activities related to the initiation and modification of medical therapeutic regimes: Prescribes medications including contraceptives and medications for sexually transmitted diseases.
* Participates in the quality assurance in the Community Health Center, assists in the development of protocols and procedures, conducts individual and group educational sessions with clients and staff and participates in the orientation of new staff and students.
* Refers patients to other services within the Northwest Colorado Health agency and other agencies, or to specialists as the cases require. Consults with staff and community physicians.
* Enters in a collaborative agreement with Providers.
* Education: Successful completion of a Medical School program including residency.
* Experience: Proven physical assessment and counseling skills for a continuum of the general population.

We offer a salary that meets market values for the Colorado area and is based on experience. The salary range for this full time (30-40 hours/week) position located in Moffat and Routt Counties is $153,400- $239,699 and is eligible for Medical, Dental, Vision, Life Insurance, Long-Term Disability, Short-Term Disability, Paid Time Off and our 403(b) Retirement Plan. POSITION HIGHLIGHTS:
* Serve as the sole MIGS specialist working within the division of urogynecology, with an exceptional opportunity to shape and grow the program while building a practice focused on complex gynecologic and robotic surgery, teaching, and future leadership as the division expands.
* Collaborate closely with the Uro/Gyn Chief to help advance the vision and strategic growth of the MIGS program.
* Enjoy no obstetric call, allowing you to focus exclusively on gynecologic surgical care within your division.
* Participate in a shared call schedule of approximately 1:4, covering one week at a time with home call.
* Access to three robotic surgical systems in a robust minimally invasive surgery environment, with strong OR and bedside support and dedicated APP support for MIGS patients, including postoperative care and follow-up.
* Faculty appointment at UMass Chan Medical School – Baystate, with opportunities to teach and mentor Uro/Gyn fellows, UMass Ob/Gyn residents, and medical students, as well as protected time for teaching and other academic activities.

This is a Part Time (5 hrs per week) position and must be on-site.
The Psychiatrist provides comprehensive mental health care to patients in a correctional setting, including psychiatric evaluations, diagnoses, treatment planning, and medication management. This role collaborates with the multidisciplinary healthcare team to ensure integrated, evidence-based care. The Psychiatrist will maintain accurate medical records, ensure compliance with regulatory standards, and support the overall mental health program within the facility.
How YOU can Make an Impact:
Provide high-quality, evidence-based psychiatric care to inmates.
Provide psychiatric crisis intervention for inmates experiencing acute mental health episodes or emotional distress. Ensure timely and effective treatment to manage self-harm risks, suicidal ideation, and aggressive behaviors.
Work closely with other healthcare professionals and correctional staff, to ensure coordinated care for patients with complex mental health and medical needs.
Provide mental health education to inmates, correctional staff, and other medical personnel, to help reduce stigma of mental health treatment and improve understanding of psychiatric disorders.
Document promptly and properly.
Provide specialized care for inmates with mental health conditions, including those with co-occurring substance use disorders, traumatic brain injuries, or histories of abuse. Address the mental health needs of high-risk or vulnerable populations such as those in segregation or those with suicidal ideation.
Conduct regular risk assessments for suicidal ideation, violent behavior, and other potential threats to inmate or staff safety. Recommendation protocols for safety and initiate involuntary treatment as necessary.
Uphold the highest standards of psychiatric practice, including ensuring informed consent for treatment, respecting patient confidentiality, and adhering to ethical guidelines set forth by psychiatric governing bodies.
Participate in continuous quality improvement (CQI) efforts, including audits, reviews, and feedback to improve patient care and ensure compliance with mental health laws, regulations, and facility standards.
Provide supervision and/or collaboration to Psychiatric/Mental Health Nurse Practitioners where needed.
Participate in training and educational initiatives for correctional staff, helping them recognize signs of mental illness and understand how to appropriately interact with inmates with mental health issues.
Provide Call-by-Phone coverage as needed, including after-hours psychiatric care for inmates experiencing psychiatric crises or severe distress.
What we Ask of YOU:
A current, valid, and unrestricted license to practice medicine in the State of Washington.
Board Certified preferred; Board Eligible welcomed. Must have completed an accredited Residency in Psychiatry.
A current, valid DEA Registration, or be willing to obtain Registration.
At least 3-5 years of clinical practice; plus, 1-2 years in correctional healthcare; clinical rotations in Residency/Fellowships count to satisfy this requirement.
Experience in diagnosis, treatment planning, and providing healthcare services in a managed care or correctional facility setting is highly preferred.
Active CPR/BLS certificate required prior to Start Date (American Heart Association/American Red Cross guideline approved).

GENERAL DESCRIPTION OF POSITION
The Cardiologist is responsible for performing, supervising, and interpreting non-invasive cardiovascular diagnostic studies, including transthoracic echocardiograms (TTE), stress echocardiograms, and nuclear stress tests. The position supports the delivery of high-quality cardiovascular imaging services by ensuring accurate diagnostic interpretation, adherence to clinical standards, and collaboration with physicians, technologists, and clinical staff to provide safe, efficient, and patient-centered care.
Schedule: Part-Time, 1 day per week (on-site)
Classification: Independent Contractor (1099)
Compensation: $20.00–$250.00 per study, depending on the type and complexity of the examination. Compensation is negotiable based on experience, credentials, and scope of services and is paid on a per-study (per-click) basis.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Perform, supervise, and interpret transthoracic echocardiograms (TTE), stress echocardiograms (exercise and pharmacologic), and nuclear cardiology studies.
Review physician orders, patient medical history, and clinical indications to determine the appropriateness of cardiovascular diagnostic testing.
Interpret cardiovascular imaging studies and prepare accurate, timely reports for referring healthcare providers using the facility's electronic medical record (EMR) and cardiovascular reporting systems.
Provide physician supervision of exercise and pharmacologic stress testing in accordance with applicable federal and state regulations, facility policies, and accepted standards of medical practice.
Evaluate patients before testing to identify contraindications and ensure appropriate patient selection and preparation.
Respond to and manage medical emergencies or adverse reactions that may occur during stress testing or imaging procedures, following current ACLS protocols.
Collaborate with cardiac sonographers, nuclear medicine technologists, nursing personnel, and administrative staff to ensure high-quality diagnostic studies and efficient patient workflow.
Ensure compliance with protocols established by the American Society of Echocardiography (ASE), American Society of Nuclear Cardiology (ASNC), American College of Cardiology (ACC), and applicable clinical practice guidelines.
Participate in quality assurance, peer review, accreditation activities, protocol development, and continuous quality improvement initiatives.
Maintain complete, accurate, and timely documentation of patient encounters, imaging interpretations, and clinical recommendations.
Communicate significant or urgent findings promptly to referring providers in accordance with facility policies.
Maintain all required medical licenses, certifications, registrations, and professional credentials.
Perform other duties within the scope of licensure and clinical privileges as assigned.
QUALIFICATIONS
Required:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.
Completion of an ACGME-accredited Internal Medicine residency and Cardiovascular Disease fellowship.
Board Certified or Board Eligible in Cardiovascular Disease (ABIM) or equivalent.
Active, unrestricted New Jersey medical license.
Current BLS and ACLS certification.
Demonstrated expertise in interpreting echocardiography, stress echocardiography, and nuclear cardiology studies.
Preferred:
National Board of Echocardiography (NBE) certification.

What You'll Do
Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria's Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
Your Qualifications
Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty.
