Job Description:
The Coder I is responsible for abstraction and assigning valid CPT, ICD-10, HCPCs codes and modifiers to ensure appropriate reimbursement in accordance with federal state, and private health plans as well as organization and regulatory guidance. This role is typically responsible for less complex coding with oversight.
Essential Functions:
- Accurately abstracts information from the service documentation, assigns and sequences appropriate CPT, ICD-10, and HCPCs codes into the appropriate billing systems, ensuring compliance with established guidelines
- Reviews and resolves coding denials
- Completes charges sessions in the assigned work queues in a timely manner
- Completes documentation meeting the current EM Guidelines for providers
- Ensures documentation meets the Teaching Physician Rules as mandated by CMS and ULH Policies prior to release of a code for billing
- Ensures documentation for Advanced Practice Providers meets the payer-specific rules prior to release of a code for billing
- Provides comments/suggestion relative to weak areas identified in the coding reviews
- Provides trending deficiencies to Senior Manager and Compliance Educator, as appropriate
Other Functions:
- Meets or exceeds organizational coding production and quality standards
- Participates in special projects and completes other duties as assigned
- Maintains daily/weekly communication with office managers, department, and providers.
- Ability to work within a team environment and meet monthly goals
- Maintain compliance with rules and regulations regarding coding
- Responds in a timely manner to questions from manager, providers, department, and representatives
- Maintains compliance with all company policies, procedures and standards of conduct
- Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
- Performs other duties as assigned
Additional Job Description:

- High school diploma or GED/equivalent (required)
Experience:
- One to four (1-4) years physician coding experience (preferred)
Certification:

Minimum of one (1) year current Operating Room experience
Principal  Accountabilities
Assists anesthesiologists as directed, performing duties according to the age/needs of the patient/patient population served.
Prepares anesthesia machine and cart with required supplies prior to each case; assists in maintaining anesthesia and monitoring equipment/ancillary devices, warmers (fluid & patient), accuracy and/or the adjustment or exchange of monitors, ventilators and gas cylinders as indicated.
Demonstrates an ability to recognize specialty needs and anticipate preparation and follow-up i.e. CJD, MH, Airborne-TB, Latex sensitivity and Laser cases.
Demonstrates the ability to assist with endotracheal intubation and extubation procedures during routine and emergency procedures.
Performs inventory; orders and restocks anesthesia medications, trays and supplies; restocks anesthesia carts according to established protocol; cleans and restocks anesthesia machines, cart, supply rooms, shelves, and refrigerators as required; transports anesthesia equipment/supplies to other departments as needed.
Ensures clean to dirty sanitation/containment and standard infection control precautions and professional performance. This includes but not limited to: vigilant sterile technique and care with invasive lines, facilitate/assist anesthesiologist in following CDC guidelines for Central Line Placement-Deterring CRBSI Catheter-related blood stream infections, disinfection and high level disinfection of semi-critical items and care of flexible scopes and specialty instrumentation.
Actively participates in Quality Assurance and associated documentation.
Other duties may be carried as patient care dictates, such as but not limited to: patient hair removal, blood product & blood cooler transport, patient transport, anesthetized patient positioning within national safety goals and MHHS RED Rules and Surgical Safety Checklist.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

Essential Functions
- Participate in procedure room preparation and post procedure turn around.
- Develop specific knowledge with regard to invasive, diagnostic, and interventional supplies which includes assisting in inventory control. Develops specific knowledge of applications, proper use and deployment techniques including documenting hemodynamic recordings, calculations, and interpretations.
- Provide patient education and emotional support for patients and their family members; ensure patient comfort. Foster teamwork and cohesiveness with team members and physicians.
- Develop knowledge specific to cardiopulmonary pathophysiology, pharmacology, anatomic anomalies, and treatment options. Assess the condition of the patient, document, and inform physician of deviations from normal criteria throughout the procedure.
- Understand and apply principles of x-ray generation and safety. Understand and demonstrate the correct operation of imaging equipment.
- May mix and administer medications under the direction and supervision of the physician (RCIS or RCES is required to pass medications). Pursuant to state laws.
- At the end of the procedure, may apply closure device, suture the sheath in place, or remove the sheath and apply pressure to obtain hemostasis.
- Provide thorough and accurate documentation of each procedure and associated events.
- Develop skills in operating, calibrating, and troubleshooting equipment in the Cardiac Catheterization Laboratory. Perform point of care testing and QA.
- Accountabilities: Scrub, circulate, and monitor in diagnostic, interventional, and CRM procedures according to scope of practice documents. Complete procedure data entry; acquire and process imaging studies, hemodynamic assessment and calculations, intra-procedure CathPCI registry requirements, and prepare procedural reports.

$20.80 - $31.20Major Responsibilities:- Sets up monitoring equipment in the operating room. This includes the operating table, autotransfusion system, electrocardiograph (ECG) and temperature cables, hyper/hypothermia blanket, medication and pressure manifolds, rapid infusion warmer, recorders, defibrillators, the patient monitoring system, and transesophageal echocardiography imaging unit (TEE) machines.
- During surgical procedures, assists in the insertion of arterial pressure lines, central venous and Swan-Ganz catheter insertion, draws arterial and/or venous blood samples for diagnostic testing, checks ventilators for proper operating efficiency, operates monitor and recording systems, and identifies the various types of intubation blades and fiberoptic bronchoscopes as well as any other special equipment or supplies required for difficult intubation.
- Calibrates all pressures and recording systems within the patient monitoring system to ensure and maintain precise readings.
- Assists in transfers of patients to and from the operating room, collects all data sheets and transports them with the patient, and establishes ECG and pressure monitoring at the destination.
- Cleans all non-disposable equipment after each procedure, performs daily maintenance checks and stocks equipment.
- Moves anesthesia equipment to proper surgical suite; checks equipment to verify proper connections, gases turned off, etc.
- Maintains adequate supply of proper equipment such as: needles, syringes, air-ways, endotracheal tube (ET) tubes, scopes and blade, stethoscope, blood pressure cuff and other necessary items in anesthesia carts.
- Troubleshoots equipment malfunctions or failures and takes all necessary steps to promptly return the equipment to a safe and reliable operating condition, using backup equipment when necessary.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
