*Position Summary: *Plans, organizes, and conducts occupational therapy services in skilled nursing environment to facilitate development and rehabilitation of mentally, physically, or emotionally disabled persons by performing the following duties personally. Responsible for ensuring that all critical tasks are fulfilled on a timely basis.

*Responsibilities*

* Conducting patient evaluations and developing individualized treatment plans
* Administering therapeutic exercises and modalities
* Documenting patient progress and communicating with other healthcare professionals
* Providing education to patients and their families on injury prevention and rehabilitation techniques

*Qualifications*

* Current state license as an Occupational Therapist
* Minimum 1 year of experience in a skilled nursing facility setting, but willing to train the right person
* Strong communication skills
* Ability to work independently and as part of a team
