We are seeking a highly motivated and skilled Physical Therapist to join our team at a Skilled Nursing Facility. As A Physical Therapist, you will be responsible for evaluating and treating patients with various physical conditions, including byt not limited to, neurological, ortho pedic, and cardiopulmonary disorders.

*Responsibilities*

* Conducting patient evaluations and developing individualized treatment plans
* Administering therapeutic exercises and modalities
* Documenting patient progress and communicating with other healthcare professionals
* Providing education to patients and their families on injury prevention and rehabilitation techniques

*Qualifications*

* Current state license as a Physical Therapist
* Minimum 1 year of experience in a skilled nursing facility setting, but willing to train the right person
* Strong communication skills
* Ability to work independently and as part of a team
