*Responsibilities:*
* Provide primary care services to a diverse patient population
* Build long-term relationships with patients and families

* Maintain accurate and timely documentation
* Engage in care coordination and patient education
*Qualifications:*
* Licensed Nurse Practitioner or Physician Assistant in Rhode Island
* Minimum of 1 year experience in a primary care setting preferred
* Passion for patient-centered care and strong communication skills
* Interest in a long-term position with a community-based practice
*Schedule:*
* Monday-Friday
* Limited on call rotation
Job Type: Full-time
Pay: $125,000.00 - $150,000.00 per year
