SummitWest Care is seeking a compassionate and skilled Physical Therapist to provide one-on-one care throughout Mesa County.Home health offers the opportunity to focus on meaningful patient outcomes rather than productivity quotas. Responsibilities

Evaluate patients and develop individualized, person-centered plans of care

Provide skilled physical therapy services in patients' homes

Improve mobility, strength, balance, and functional independence

Educate patients and caregivers regarding treatment goals and home exercise programs

Monitor progress and adjust treatment plans based on patient needs and goals

Complete accurate, timely clinical documentation

Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to achieve optimal outcomes

Qualifications

Current Colorado Physical Therapist (PT) license in good standing

Graduate of an accredited Physical Therapy program

Valid driver's license, reliable transportation, and auto insurance

Strong clinical evaluation and treatment skills

Excellent communication and documentation abilities

Ability to work independently while collaborating effectively with an interdisciplinary team

Ability to adapt professionally in a home-based care environment
