*Job Summary*
We are seeking a dedicated and compassionate Physical Therapist to join our team, providing high-quality care to patients in a home health setting. *Duties*
* Collaborate with healthcare professionals to coordinate patient care and ensure treatment goals are met
* Conduct comprehensive assessments of patients' physical conditions and functional abilities
* Develop individualized treatment plans tailored to each patient's needs and recovery objectives
* Implement therapeutic interventions, including exercises, manual therapy, and other modalities
* Monitor patient progress and adjust treatment strategies accordingly
* Educate patients and their families on therapy techniques, injury prevention, and health maintenance
* Maintain accurate documentation of patient assessments, treatment sessions, and progress notes in compliance with regulatory standards
* Provide home health services, ensuring safety and comfort during therapy sessions
* Adhere to all safety protocols and maintain CPR certification at the time of employment
*Experience*
* Graduate of an accredited Physical Therapy Program from an American Physical Therapy Association (APTA) recognized school, with a BS degree or entry-level master's degree
* Active Connecticut PT license is required
* CPR certification must be current at the start of employment
* Prior experience in home health settings is preferred but not mandatory; strong communication skills and a collaborative attitude are essential for success in this role
