*What You'll Do*

* Conduct Start of Care visits and deliver hands-on in-home OT treatment
* Build and manage individualized care plans
* Collaborate closely with an interdisciplinary care team
* Document in HomeCare HomeBase (HCHB)

*What You Bring*

* Active Colorado OT license
* Home health experience preferred (not required)
* Strong communication and documentation skills
* Valid driver's license, auto insurance, current CPR certification
