*Job Overview*
We are seeking a dedicated and compassionate *Nurse Practitioner (NP) or Physician Assistant (PA)* to provide high-quality primary care services to patients in *skilled nursing facilities (SNFs)*. This is a *field-based role* focused on delivering comprehensive, patient-centered care in post-acute and home settings. The ideal candidate is self-motivated, comfortable working independently, and passionate about improving healthcare access for vulnerable populations.
*Key Responsibilities:*
* Conduct in-person medical visits at assigned *Skilled Nursing homes.*
* Perform *routine and acute care evaluations*, chronic disease management, preventative screenings, and medication management
* Collaborate with *facility staff, mobile medical assistants (MAs)*, and telehealth support personnel to ensure coordinated and efficient care
* Document clinical encounters thoroughly and timely using *EHR systems*
* Order and interpret *labs, diagnostics*, and develop comprehensive *care plans*
* Communicate with family members, caregivers, and interdisciplinary teams as needed
* Adhere to evidence-based guidelines, organizational protocols, and state/federal regulations
* Participate in regular case reviews, team meetings, and quality improvement initiatives
* Have current DEA
