COMFORT HOME HEALTH is currently looking to hire an RN, Case Manager for Howard County and surrounding areas.

Education/Skills

* Graduate of an accredited school of nursing
* 1 yr. Experience (Homecare preferred)
* Valid Registered Nursing license with the State of Indiana
* Communication Skills
* Time Management
* Competent with use of PC

Responsibilities: Providing skilled professional nursing care to patients per physician's order and in compliance with regulations established by the Indiana State Board of Nursing and Agency policies.

* Patient Assessment
* Initiates Plan of Care
* Document efficiently & effectively
* Prepares Clinical and Progress Notes
* Care Coordination
* Patient Education
* Supervisory Visits
* Performs Skilled Nursing tasks as ordered by the Physician on Plan of Care
* Blood Draws
* Med Sets
* Specimen Collection
* Wound Care
* Ostomy Care

Must have reliable transportation, valid Indiana Driver's License, current Auto Insurance

Must pass National Background Check
