*Overview*

*Duties*

* Conduct comprehensive evaluations of patients’ physical conditions, including mobility, strength, flexibility, and functional abilities.
* Develop and implement tailored treatment plans utilizing physiotherapy techniques such as trigger point therapy, orthopedics interventions, neurology strategies, and geriatrics-focused therapies.
* Manage discharge planning processes by coordinating with healthcare professionals to ensure seamless patient transitions from hospital to home or community settings.
* Document clinical assessments, treatment progress, and care plans accurately in electronic health records (EHR) systems following clinical documentation standards.
* Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes through multidisciplinary care.
* Educate patients and their families on treatment exercises, injury prevention strategies, and home & community care practices to support ongoing rehabilitation.
* Monitor patient progress regularly and adjust treatment plans accordingly to meet evolving needs in outpatient clinics or inpatient environments.

*Qualifications*

* Valid state licensure or certification as a licensed Physical Therapist with practicing in a physical therapy role.
* Proven experience in acute care settings such as hospitals or inpatient units is highly desirable.
* Demonstrated expertise in outpatient rehabilitation center environments or outpatient clinics with knowledge of outpatient experience requirements.
* Strong understanding of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to orthopedics, neurology, pediatrics, and geriatrics.
* Experience working with geriatric patients and familiarity with home health or community-based care programs.
* Proficiency in electronic health records (EHR) management systems (EMR systems) for documentation and treatment tracking.
* Ability to develop comprehensive care plans that include discharge planning and patient rehabilitation strategies.
* Excellent client communication skills to effectively educate patients about their treatment plans and foster engagement in their recovery process.
